WI-85-036-002 The basis for Element Seport MC-40510-SQN, Rev. 2, dated October 21, 1986 is Watts Bar Employee Concern WI-85-036-002 which states:
"Material requisitions are not being prepared, as per procedure; the requisitions are being changed or altered later (CI will not civulge additional information."
This concern was evaluated by TVA as potentially nuclear safety-related and potentially applicable to Sequoyah (generic).
II.
Su-mary of Issue The issue defined by TVA is that if purchase rec isitions are being changed or altered later (presumably subsegrent to review and approval),
it is possible that the changes or alterations were not properly reviewed In this case, inadequate material could and approved (concurred with).

III. Evaluati0n TVA personnel determined that Sequoyah Administrative Instruction AI-11 sets the recuirements for changing or altering quality-related procure-Pent documents.
A review by TVA of a random sample of site-initiated purchase recuisitions did not identify any discrepancies.
Interviews performed by TVA during the course of the investigation identified one change to a purcnase requisition without proper review and acproval; the discrepancy was identified during the site receipt inspection and documented on a Nonconforming Item report.
The TVA evaluation concluded that AI-11 is adequate in respect to controi-ling improper changes or altera' ions to quality-related purchasing docu-ments.
The one discrepancy identified appears to have been a singular occurrence, and was not indicative of any systematic problem.
No corrective action is required.
IV.
Conclusions The NRC staff believes that the TVA investigation of the concern was adequate, and their resolution of the concern as described in Element Report MC-40510-SQN, Rev. 2, is acceptable.
An NRC procurement inspection at Sequoyah during the periods September 15-19, 1986 and September 29 -
October 3, 1986 did not identify any examples of improperly changed or altered procurement requisitions.

Velding - Crncerns about any aspect of welding, including welder or weld procedure cud ification, weld inspection / nondestructive examination, heat treatment, weld quality, filler material quality, and weld documentation.

Intimidation, Harassment, Wrongdoing, or Misconduct - concerns about personnel conduct that interferes with the ability of employees to fulfill their assigned responsibilities, unauthorized actions taken against employees for fulfilling their assigned responsibilities, and illegal activit4s or violations of TVA policies and regulations.

Management and Personnel - Concerns about the adequacy of policies, ranagement attitude and effectiveness, organization structures, personnel management, and personnel training and qualification, except training and ovalification covered by the Ouality Assurance /Ouality Control Category.

Quality Assurance / Quality Control - concerns about the adequacy of Quality Assurance / Quality Control programs and procedures (e.g.,
