No. 12 Steam Generator Steam Flow Channel 2 - Inoperable This report was initiated by Incident Report 82-070.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 3

.Rx Power 0% - Unit Load 0 MWe.

DESCRIPTION OF OCCURRENCE:

On April 5, 1982, the Control Room Operator noticed that the indication for No. 12 Steam Generator Steam Flow Channel 2 had failed high.

The channel was declared inoperable, and at 0655 hours0.00758 days <br />0.182 hours <br />0.00108 weeks <br />2.492275e-4 months <br />, Limiting Condition for Operation 3.3.2.1 Action 14 was entered.

This constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The high steam flow indication was due to leakage from a loose tubing fitting on the flow transmitter.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.2.1 Action 14 requires:

With the number of operable channels one less-than the total number of channels, operation may proceed until performance of the next required channel functional test, provided the inoperable channel is placed_in the tripped condition within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />.
