Air Particulate Detector Pump - Inoperable.

This report was initiated by Incident Report _82-011.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 6 - Rx Power 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:

On January 13, 1982 an operator discovered that the Air Particulate Detector (APD) Pump had failed.

An attempt to restart-it was un-successful.

Therefore, at 0001 hours1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, the APD Pump was declared inoperable, rendering radiation monitors 1RllA, 1R12A, and 1R12B inoperable, and Action Statements 3.3.3.1.b Action 22 and 3.9.9 were entered.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The high speed and low speed filter paper drives had failed.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.3.3.1.b Action 22 requires:

With the number of channels operable less than required by the minimum channels operable requirement, comply with the action requirements of Specification 3.9.9.

Technical Specification 3.9.9 requires:

With the containment purge and pressure-vacuum relief isolation system inoperable, close each of the purge and pressure-vacuum relief penetrations providing direct access from the containment atmosphere to the outside atmosphere.

LER-82-06/03L. 'CORhECTIVE' ACTION:

All of the required containment purge and pressure-vacuum relief penetrations were isolated.

The high speed and low speed filter paper drives were replaced and the APD-was tested satisfactorily.
