Transmittal of Problems with Selected Element Reports in the Operations Category for Employee Concerns During our review of your Operations Category Enployee Concern Element Reports, the staff has (in some cases) had various problems associated with the resolution addressing the specific concern, the scooe of the investigation of the concern, the corrective actions required and the cause or reason for the problem that initiated the concern.
Enclosed is a listing of element reports with exerples of staff problems.
It is requested that these elcrent reports be reviewed and appropriately modified.
In our continuing review we will provide further comments regarding similar problems with other element reports.
In the meantime you should consider these types of problens in your continuing review of element reports.

Element Report Cnmnents 301.17 - Final Report - Systen 31 (1) The element report makes not Operated Properly assumptiers based on verbal information from employee interviews without confirr.ing that infcrmation using indepth document reviews.
(?) The element report did not evaluate why failures of switches did ret occur at Sequoyah as they did at Watts Ear. This might simply be a training issue ard rot c hardware issue. klithout that knowledre, there is no assurance that the issue will not occur in the future at Sequoyah.
301.05 - Final Report -
(1) The welding issue of pro-Questionable Design and Cerstruction tecting items frcm weld Practices splatter was not considereo.
(2) The investigation failed to realize that, in the past, rotter Feses have been used to transfer bottoms and not just distillate.
(3) Since the concerned freivic'tril stated be was concerned about radiation frem tFr Fose, some determination shculo have been rede abrut personnel contami-nation hazards.
(4) The distillate was uncorrectly stated tc tc rerradioactive.
Sufficient raoicactivity exists in the liquid to certaminate personnel.

. Element Report Corrrnents (5) The dispute between line orgar-izations regarding the adecuary of limit switch mountings is not addressed in the element report.
(6) Similar nounting concerns (see 5 above) were not setisfactorily addressed f rom a corrective action perspective.
(7) No cause as to why the switches v.'cre not clearly mounted correctly ves given.
308.06 - Final Report -
(1) TVA, in their evaluation, has Subjcurneyman/ Journeyman assumed the issue to be a labor relations issue at Watts Bar (which is not reall cable to Sequoyah) y appli-instead of an issue of work being done by unqualified personnel.
(2) The element repo N assumes that, because procedures are in place at Sequoyah for on the job trainirn of sub-journeyman, all work was done properly, flo attempt was it.ade to determine if unqualified personnel had done work re-quired to be dere by personnel with special qualifications or that the on the job treining procedures were adequate and being properly implemented.
308.01 - Final Report -
Concerning MAS-86-001: Contradictory Adequacy of Precedures statements to the conclusion in the elemert rcpcrt were found in the document back-up files indicating that craft personnel were having difficulty interpreting MI C.EO.
Herever, an engineer concluded the procedure was adequate.
The element report did not evaluatt actual irolementations of MI 6.70 by craft personnel f rom docurcents generated by previous use of the procedure.

(1) The element repcrt did ret Accessibility evaluate whether FSAR coninit-ments were continuina to be net. l'odifications have been trade to the plant. Such modificatiers right have impacted the cperators access-ibility to plant equipment since the original deter-raination of adequate operator accessibility.
(?) Corrective action was not identified for programs which have shown little progress.
301.07 - Final Report -
(1) The elen,ent report did not General Paint Concern address S.G. Pinney and Reactor Buildirg Associates, Inc, recommendations.
(2) All aspects of the safety sigrif ficence of the paint chips were riot evaluated in the element report.
(3) PRO 1-86-139 was evalueted as not reportable. CAR 86-01-001 was identified es a safety-related issue judged to be rcn-significant. The report (ces rot address the factors which supported these oeter-minations.

(1) The element title does not Possible lack cf Patertight aDpear appropriate to the Conduit and Conr.ections discussion since the discussion deals with the Raychem splices for electrical ceF1c, The discussion has nothing to do wIth conduits and connections unless the term " connections" is used to trean cable connections.
(2) The discussion in the rccert does not mention the extensive enstronrrental qualification testing and inspectier efforts that the Sequoyah EQ project is perfonning to ensure that "as installed" Paychem splice configurations are qualified for use in 10 CFR 50.49 applications. This effort was undertaken since splice con-figurations different than Raychem recontr. ended and qualified were identified durir.g plant E0 inspection activities.
Discussion of these ectivities is considered appropriate to this element report are tFe rctolution of the related en:ployee concern.
(3) The imedietc t.nd long-tena corrective actions should, as a minimum, includc the establishment of a program to inspect 100', of the Pe.ycher.1 applications covered by 10 CFR 50.49, and include the iripleroentation of procedural cherces tFat ensures adequate documentation and traceability in the work rackeces that adequately describes the configuraticn in the ficid.
