Dear Sir:

Attached is Revision 1 to Licensee Event Report No. 81-20, concerning failure l

of the "B" Train Control Room Chlorine Detector unit. The corrective action originally taken to prevent binding of the motor shaft was apparently insuffi-cient as the same failure occurred again two months later. On this occasion, the corrective action taken was to replace the entire blower motor. The "A" Train Chlorine Detaction System was operable at all times, therefore, there was no effect on plant or public health and safety, a

Facility: Trojan Nuclear Plant, P.O. Box 439 Rainier, OR 97048 4.

Identification of Occurrence:

During a routine plant tour, an Auxiliary Operator noticed that the blower to the Control Room "B" Chlorine Detector was not operating properly. Two months later, the same blower was discovered to have failed again.

Conditicns Prior to Occurrence:

i The plant was in Mode 1 at 1007. power and steady state operation on both occasions.

Description of Occurrence:

l The Chloralert Blower Ibtor on the Chlorine Detector was not working properly, which rendered the detector inaperable. The unit is a Fischer-Porter Model 17CA1010 Chloralert Blower Motor.

Designation of Apparent Cause of Occurrence:

The electric motor on the blower appeared to be binding as lubricant 4

applied to the motor bearing would free the blower for a short time.

Analysis of Occurrence:

I Inspection showed that a dirty bearing on the blower motor was causing l

the motor to bind. There was no ef fect on plant or public safety sines the "A" Train Control Room Chlorine Detector was operable.

Corrective Action

Upon discovery, the detector was immediately taken out of service and the Contrcl Room ventilation was transferred to the emergency recirculation mode pcr S.T.S. 3.3.3.6.

The motor bearing was cleaned and lubricated, the flow metering tube was cleaned, and the detector was calibrated and returned to service on September 8,1981.

Following the seconi identical failure of the blower, the entire blower motor was replaced to prevent recurrence. Periodic inspection and cleaning of the chlorine Detector units has been added to the prevantive maintenance program.
