In accordance with the Trojan Plant Operar'ng Lice endix A, US NRC Technical Specifications, Paragraph 6.9.1.7, we are submitting Licensee Event Report No. 81-15.

This report describes a situation where the Technical Specification for containment isolacion valves was not satisfied due to an inoperable valve.

REPORTABLE OCCURRENCE 1.

Report Date:

Occurrence Late: July 28, 1981 3.

Facility: Trojan Nuclear Plant, PO Box 439, Rainier, OR 97048 4.

Identification of occurrence:

Containment isolation valve MO-2813 for "B" Steam Generator Blowdown was found failed in its open position. This valve is required to function to maintain Contrinment integrity per Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

The plant was operating in Mode 1 at 80% power.

Description of Occurrence:

During routine operations the "B" Steam Generator Blowdown Containment I

Isolation Valve M0-2813 failed to close. Technical Specification 3.6.3.1 requires this valve b; aperable and capable of being auto-matically ehut by a containment isolation signal.

In its failed conditien, the valve could nar meet the requirenent.s for Containment integrity.

Designation of Apparent Cause of 0 c; :cace:

The cause of this event is attributed to motor fail *.:re of the valve nperator.

Ineulation breakdown cause the motor to short out.

Significance of Occurrence:

This event had no adverse effect on either plant or public safety.

Cor.cainment integrity was met by shutting the redundant valve for this containment penetration as required by the limiting condition for operation.

Corrective Action

Immediate corrective action was to close the redundant valve for the Containment penetration. The failed motor was then replaced.

Review of Equipment History Files showed no previous motor failures on safety-related valves in the Blowdown System.
