Licensee Event Report No. 81-17, concerving a situation where "C" SI accumulator was not sampled for boron coacentration after increasing its level by 11%. This could have led to a possible reduction in shutdown capability on ESF actuation. Technical Specification Paragraphs 3.5.1.c and 4.5.1.b require sampling the SI accumulators within six hours of increasing the liquid volume by 1% of tank volume to verify that the boron concentration is greater than 1900 ppm.

This is a 30-day reportable occurrence.

Identification of Occurrence:

Af ter increasing level in "C" SI accumulator by 11%, the boron concentration was not verified within six hours as required by Technical Specification 4.5.1.b.

Conditions Prior to Occurrence:

The plant was at 80% power, steady-state operation with an average RCS temperature of 578'F, pressurizer pressure of 2238 psig, and four RCPs running.

Description of Occurrence:

On August 1, 1981, a review of the Operations Log indicated no boron concentration for "C" SI accumulator had been logged af ter filling the accumulator on July 31, 1981. The previous sarv,' s was taken on July 30, 1981. After increasing level by 11% from tzra RWST, the operator notified Chemistry to draw a boron sample. The request was apparently not transferred to the oncoming shift and no concen-tration was logged.

Designation of Apparent Cause of Occurrence:

Plant Review Board minutes classified the cause as " observed l

inadequacy in implementation of administrative controls." This occurrence was caused by personnel error. The Chemistry and Operations Departments failed to follow up on the request for a sample to verify that the sample had been obtained.

Analysis of Occurrence:

Since the accumulator was filled f rom the RWST (boron concentration greater than 2000 ppm) and samples taken before and after filling indicated boron greater than 2100 ppa, the plant was not operated in a degraded mode.

Corrective Action

A sample was taken immediately upon discovery of the occurrence 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> and 38 minutes after filling the accumulator. The import-ance of ensuring that the requested sample is received and logged has been stressed to plant personnel.
