Dear Mr. Stello:

During a routine shutdown of the General Electric Test Reactor (GETR) on May 12, 1977 it was discovered that a skirt section of the reactor pool cover had not been put into place during the previous reactor shutdown. Accordingly, the reactor was operated at full power for approximately seventeen days without the skirt section in place. This occurrence is considered reportable pursuant to paragraph 3.D.(2) of License TR-1.

During the shutdown, the missile shield which covers the reactor pool was removed, and it was noted that the section of the pool cover skirt which attaches to the gates between the reactor pool and the reactor canal was not in place. Nomally during a shutdown, the gate section of the skirt is removed and replaced with a ladder to allow access to the pressure vessel head. After the pool is raised to the third floor Icvel for the last time, the ladder is removed and the skirt section is replaced.

The step in the Shutdown Sequence list which describes the required action is surded as follows:

" Raise Pool Level. Remove portable ladder and install skirts on canal gates while pool is raising".

The sign off and verification for this step cover u,o separate operations, one required to be performed in the control room and the second perfomed on the third floor of the reactor containment building. Although the step had been signed off as completed by the employee in the Control Room, the skirt section had not been* replaced.

9 GENER AL h ELECTRIC Mr. Victor Stello June 10, 1977 In order to prevent recurrence of this problem, the Shutdown Sequence list has been revised to separate the tw operations and to require an individual sign-off for each of the operations.

In addition, a sign-off was added to verify that all skirt sections are in place prior to the installation of the pool cover.
