Gentlemen:
This refers to the inspection conducted by P. H. Harrell of this office during
the period December 1-31, 1986, of activities authorized by NRC Operating
License DPR-40 for the Fort Calhoun Station, and to the discussion of our
findings with you and other members of your staff at the conclusion of the
inspection.
Areas examined during the inspection included operational rafety verifications,
maintenance, surveillance, plant tours, safety-related system walkdowns,
security observations, inoffice review of periodic and special reports,
followup on previously identified items, cold weather preparations, and
followup on an allegation related to dismissal of an individual for
incompetence. Within these areas, the inspection consisted of selective
examination of procedures and representative records, interviews with
personnel, and observations by the inspector. The inspection findings are
documented in the enclosed inspection report.
During this inspection, it was found that certain of your activities were in
violation of NRC requirements. Consequently, you are required to respond to
these violations, in writing, in accordance with the provisions of
Section 2.201 of the NRC's " Rules of Practice," Part 2, Title 10, Code of
Federal Regulations. Your response should be based on the specifics contained
in the Notice of Violation enclosed with this letter. Since it was also found
during this inspection that you have taken corrective action, inc19 ding action
to prevent recurrence, no response to Violation C need be submitted.
Violation B revealed the continuing failure to adequately control modification
activities, in that.this finding is similar to previous concerns identified by
the NRC during the Safety System Outage Modification Inspection completed in
early 1986.
Prompt management attention is needed regarding this matter.
We have also examined actions you have taken with regard to previously
identified inspection findings. The status of these items is identified in
paragraph 2 of the enclosed report.
