The purpose of Requalificatio Program (tune J usea to ensure ITiaT. current license holders at Crystal River Unit 3 (CR-3) maintain a high level of technical competence and awareness of plant and procedural changes.

2.0 SCOPE The procedure addresses the training requirements necessary to be completed prior to renewal of NRC licenses as well as reactivation of licenses that m3y have become inactive at CR-3.

The procedure for Certified Instructor Requalification is TDP-209. The license renewal process as well as interfaces with FPC Licensing are described in TDP-210.

This program meets or exceeds the requirements of 10CFR55 Appendix A, ANSI N18.1-1971, and FPC commitments.

4.0 DEFINITIONS 4.1 REACTOR OPERATOR - Any individual who manipulates a control of a facility. An individual is deemed to manipulate a control if he directs another to manipulate a control.

4.2 SENIOR REACTOR OPERATOR - Any individual designated by FPC to direct the licensed activities of licensed operators.

LICENSED OPERATOR RE0VALIFICATION TRAINING PROGRAM Unsatisfactory performance on two Semi-Annual Evaluations during a license period or two consecutive evaluations if in different license periods.

Unsatisfactory or incomplete performance of Annual and Two-year Manipulations.

Unsatisfactory performance during Simulator Training.

Not performing licensed duties for four months.

While in Accelerated Requalification Training, the licensee will not perform any licensed duties.

This training is specifically structured to rapidly upgrade the licensee's knowledge and/or skills in the deficient area (s). Accelerated Requalification Training includes additional training requirements, schedule for completion, and testing criteria, which must be completed satisfactorily prior to resuming licensed duties.

5.0 RESPONSIBILITIES 5.1 VICE PRESIDENT NUCLEAR OPERATIONS - Responsible for certifying to the j

g NRC that the licensed operator has competently and safely discharged assigned licensed duties.

5.2 NUCLEAR OPERATOR TRAINING SUPERVISOR (NOTS) - Responsible for implementing the classroom lecture, Simulator Training and written examination portions of the Requalification Training Program.

5.3 NUCLEAR OPERATIONS TRAINING MANAGER (NOTM) - Responsible for certifying to the NRC that the licensed operator has satisfactorily completed the approved Requalification fraining Program; managing the development and implementation of the Requalification Training Program.

5.4 NUCLEAR OPERATIONS SUPERINTENDENT - Responsible for implementing the semi-annual performance evaluation phase of the Requalification Training Program for Shift Supervisors; assigning backup licensees to on-shift duty.

5.5 NUCLEAR SHIFT SUPERVISOR - Responsible for conducting and documenting portions of the on-shift training, the skills training and evaluation (Phase 2) and the semi-annual evaluation (Phase 3) requirements of the Requalification Training Program for personnel assigned to his shift including backup licensees as assigned by the Operations Superintendent.

5.6 LICENSEE - Responsible for satisfactorily completing all the l

l Requalification Training, including any Accelerated Requalification Training.

It is the Licensee's responsibility to identify to his supervisor the need for attending training.

Phase 1 - Preplanned Lecture Series.

Phase 2 - Skills Training and Evaluation.

Phase 3 - Annual Written Examination and Evaluation.

This training shall be conducted only by Instructors who meet the requirements of TDP-209, " Operations Instructor Qualification Program".

This training shall be conducted on a continuing basis, and scheduled to be completed every two years in accordance with TDP-117,

" Scheduling and Tracking Training".

The only exception to this is unplanned outages.

6.1.1 Phase 1 - Preplanned Lecture Series q

6.1.1.2 Special Training (Topic of 6.1.1.1.n) - Lectures on the topic "Special Training" address such subjects as LER's, TAP's, SER's, SOER's and other feedback of operating experience, as well as modifications and changes to plant design.

Special training lectures may be grouped into the categories listed in Section 6.1.1.2 if appropriate.

The intent of Special Training is also to provide operators with insight into planned evolutions and plant activities.

Additional Special Training lectures shall be developed from evaluations of operating experience in accordance with TDP-105 and Section 6.11 of this procedure.

6.1.1.3 Lecture Series Schedule - The preplanned lecture series shall be scheduled on an annual basis with a minimum of six lectures given each year.

These lectures shall be evenly spaced throughout the year, taking into account plant operating conditions, unanticipated events and planned shutdowns / refueling.

The lecture series shall be conducted on a continuing basis and involve at least forty (40) lecture hours of instruction for each licensed operator per year. shows a typical weekly schedule with categories to be covered during the training cycles of the annual program.

LICENSED OPERATOR RE00ALIFICATION TRAINING PROGRAM 6.1.1.4 Lecture Subject Selection - Each lecture topic listed in Section 6.1.1.1 shall be covered during the two-year requalification cycle.

The depth of coverage in each lecture topic shall reflect strength and deficiencies identified by semi-annual evaluations of performance, annual written requalification examination results and feedback from Operations, industry operating experience, NRC requirement, and FPC commitments.

Thermodynamics and heat transfer training shall be given during each two-year requalification cycle. Credit towards fulfillment of this requirement may be satisfied by lectures specified in Section 6.1.1.1.b.

Accident mitigation training shall be given during each two-year requalification cycle. Credit towards fulfillment of this requirement may be satisfied by lectures specified in Section 6.1.1.1.1.

This training shall satisfy the intent of INP0 Guidelines STG-01 Rev 1 of 1/15/81, and as a minimum address the follcding topics:

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM 6.1.1.5 Lecture Series Attendance - All licensed personnel shall attend all lectures in the program subject to the following exemptions:

a.

Licensed personnel scoring 80% or above on sections of P '

Annual Written Examination may be exempted from att lectures on topics related to that section during ext annual cycle.

This waiver must be completed in at ur.ce with TDP-112. " Training Waivers".

Attendance shall be recorded in accordance with Reference 3.8 and absences must be made up. Licensed individuals missing all presentations of a particular lecture shall make up the lectures by self-study and/or discussions with knowledgeable persons designated by the Nuclear Operations Training Supervisor or by successful completion of a quiz (oral or written) covering the lecture material missed. No more than 50% of required lecture series shall be c6mpleted through self-study.

6.1.1.6 Evaluation of Performance - Written quizzes shall be administered as I

h part of the lecture series presentation and shall test the material covered during the lectures, except that quizzes need not be given on special training topics.

The quizzes shall be graded and should be tviewed with the class prior to returning the licensees to licenaed duties.

The NOTS via the NOTM should notify the Operations Superintendent of any license holder under his supervision who obtains less than 80% on any of the written quizzes.

For licenses obtaining a quiz grade of less than 80%, a remedial training program shall be developed in accordance with TDP-113,

" Remedial Training."

The program should identify topical areas and schedules for completion. A dritten quiz shall be given as part of the remedial training with a grade of 80% or greater required.

6.1.2 Phase 2 - Skills Training and Evaluation (On-the-Job Training).

Each licensed operator shall participate in the following activities:

a.

Reactivity manipulations and plant evolutions.

b.

Simulator exercises.

c.

Operational proficiency training.

6.1.2.1 Reactivity Manipulations :nd Plant Evolutions. and 4, Annual Control Manipulations Record, and Two-year Control Manipulations respectively, identify those reactivity manipulations and plant evolutions which shall be perforced by each licensed operator either at CR-3 or at the Simulator. activities shall be performed or simulated annually. activities shall be performed or simulated at least every two years, except as follows:

Any licensed oparators unable to perform operation of the manipulator crane during core loading or unloading, due to plant operating conditions, shall receive training on fuel handling equipment and refueling procedures prior to operating or directing operation of the manipulator crane during a refueling outage.

All Senior Reactor Operators shall perform or direct performance while all Reactor Operators shall perform the activities.

12/01/85 Paae 12 of 58 TITLD LICENSED OPERATOR RE0VALIFirATION TRAINING PROGRAM Tiie Shift Supervisor shall sign Attachments 3 and 4 when the Reactor Operator or Senior Reactor Operator satisfactorily performs or directs the performance of the activity at CR-3.

The Shift Supervisor may sign his own Reactivity Manipulations Form.

The Simulator Instructor shall sign Attachments 3 and 4 when the Reactor Operator or Senior Reactor Operator performs or directs performance of the activity on the Simulator.

(in lieu of the Simulator Instructor's initials on Attachment 3 and 4, the NOTS may sign the attachments based on a written report received from the Simulator Instructors).

Satisfactory completion should include appropriate discussion of procedures used.

The NOTS via the NOTM must notify the Operations Superintendent of any licensed individual who unsatisfactorily completes or fails to complete any required reactivity manipulation or plant evolution.

The liccasea must be removed from licensed duties and not reinstated until the Accelerated Requalification Program is completed.

The NOTS shall develop an Accelerated Requalification Program in accordance with TDP-113. " Remedial Training."

The Accelerated Requalification Program should identify training topics, schedules and additional testing criteria.

Within sixty (60) days of the end of the yearly or two year cycle

' the Attachments 3 and 4, if appropriate, must be forwarded to the NOTS for documentation purposes.

If all the items are not complete, the licensed operator must be removed from shift and placed in an Accelerated Requalification Training Program.

6.1.2.2 Simulator Exercises Each license holder shall complete five days of Simulator Training annually.

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM The Simulator used shall reproduce the general operating characteristics of CR-3 and the arrangement of instrumentation and controls shall also be similar to CR-3.

The Simulator at Power-Safety International facilities in Lynchburg, Virginia, meets these requirements. Other simulators may be used on a case-by-case basis when approved by the NOTM.

The PcwerSafety Simulator Program for Requalification Training is described in Appendix A.

The Simulator Program shall consist of lectures, self-study and normal and emergency plant operations with emphasis on emergency operations and infrequent evolutions (e.g., station blackout, TAP transients,etc.).

The Simulator Training must include performance of each item in Attachments 3 and 4 that has not been performed on the plant.

Written evaluations of overall performance shall be completed at the end of training on each license holder by the simulator instructor (s) or FPC evaluators using Attachment 8 or a similar form having the same information.

These evaluations shall be forwarded to l

I the NOTS who shall review the evaluations and determine any remedial training requirements.

l The NOTS must notify the Operations Superintendent via the NOTM of any unsatisfactory simulator performance as evaluated by the Simulator Instructors during Simulator Training.

The NOTS, NOTM, Ops Sup and Ops Manager must evaluate the license holder's overall

. performance and determine whether or not the person should be removed from license duties, and if removed from license duties, the licensee must not perform any licensed duties until an accelerated requalification program is completed.

If the licensee is not removed from licensed duties, a written justification approved by the Operations Manager must be maintained.

An Accelerated Requalification Program shall be developed in accordance with TDP-113 " Remedial Training."

The Accelerated Requalification Program should identify training topics, schedules, and shall include re-examination at the Simulator for satisfactory performance.

6.1.2.3 Operational Proficiency Training This activity consists of three parts:

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM INSTRUCTIONAL REVIEW SESSIONS:

Each Quarter (January-March, April-June, July-September, and October-December) the NOTS shall issue a Quarterly Review Record (Attachment SQ1-SQ4 and SR as appropriate) which identifies the required on-shift reading and review activities for that quarter from items identified below.

The breadth of topics shall be based on a combination of Operations input, exam results, and operational experience.

The depth of coverage will be that required for an SRO.

The NOTS shall include reviews of any new EP's and AP's, or changes thereto, issued during the previous quarter on the Quarterly Review Record, Attachment SR.

The Instructional Review Sessions shall be developed such that the following topics are covered on an annual basis:

a.

Normal operating procedures (limits, precautions, and setpoints only).

b.

All abnormal procedures.

c.

All emergency procedures.

NOTE:

Procedure training shall include review of prerequisites, precautions, limitations, setpoints and procedural steps.

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM If operational activities included in the Quarterly Review requirements are not performed, they shall be walked through using appropriate procedures.

This may be satisfied by performing activities on the Simulator per Attachments 3 and 4.

The Shift Supervisor shall initial the Quarterly Review Record (Attachment SR) to indicate that the topics have been covered and understood.

The Shift Supervisor shall note any deficiencies and provide instructional sessions as necessary to correct these deficiencies.

The NOTS shall sign the Review Record for the Backup Licensees who are not Operations Department personnel.

PLANT DRILL PROGRAM All license holders shall participate in at least one plant drill quarterly.

The Jrills may include:

I Security Event Fire Drill Injured person Radiation / Contamination problem Emergency Plan Drill Operational drill conducted by the Training Staff Drills conducted on the Simulator At least one Abnormal and/or Emergency Procedure walk-through shall be conducted annually by the Shift Supervisor for his shift using the main control panel and describing what would be seen on the instrumentation.

The actions to be taken will be discussed, actual manipulation is not required.

If any weak areas are discovered while conducting drills, the Shift Supervisor must hold an instructional session to correct the deficiency. Licensed or certified SR0's designated by the Operations Superintendent will conduct at least one Abnormal / Emergency Procedure drill on the Shift Supervisors annually.

The Shift Supervisor shall sign the Quarterly Review Record when a drill is completed satisfactorily. An instructor, or other assigned SR0 evaluator, shall evaluate the Shift Supervisor's response and record it on the Review Record and sign as appropriate.

These items shall be reviewed with the shift personnel by the Shift Supervisor, Nuclear Operations Instructor, Shift Operations Technical Advisor, or other assigned SR0 personnel.

The Shift Supervisor shall sign the Quarterly Review Record when the license holder has completed reading and/or discussing the operational feedback items.

6.1.2.4 Reporting Results Within fif teen days of end of the Quarter, Shift Supervisors must submit to the NOTS the completed Quarterly Review Records for personnel on the Supervisor's shif t.

The NOTS or Nuclear Operations Instructor must review the Quarterly Re 'iew Record for completion, and sign and attach the record to the requ.fication Training Checklist (Attachment 2).

If progress is not satisfactory, the NOTS must notify the Operations Superintendent and the Training Manager.

The Operations Superintendent should review the individual's records and develop a written remedial action plan to get the individual back on schedule.

A copy of the action plan must be forwarded to the NOTS for review and recording when the action is complete.

If at the end of the annual cycle (plus one month) the individual has not completed all items satisfactorily, the NOTS mus* notify the Operations Superintendent, who must remove the operator from licensed duties.

The NOTS must develop an Accelerated Requalification Prograin which identifies training topics, schedule and additional completion criteria.

The Accelerated Requalification Training Program shall be completed satisfactorily prior to resuming licensed duties.

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM The Annual Written Exam shall be prepared, administered and graded in accordance with TOP-106 by qualified Operations Instructors or FPC Consultants who meet the requirements of TDP-209.

Satisfactory completion of the annual written examination shall require at least 70% in each category and an overall average of at least 80%. Grading must be completed within five working days of the examination date.

An individual who scores less than the above criteria shall be removed from licensed duties until an accelerated requalification program is satisfactorily completed.

The Accelerated Requalification Program must be developed in accordance with TDP-il? " Remedial Training."

The Accelerated Requalification Program should identify training topics, schedules and re-examination as follows:

Original Test Score Action Less than 80% overall test of all 4 categories Less than 70% in one category test of that category Less than 70% in two or more categories test of all four categories The NRC shall be notified when an individual has satisfactorily completed the Accelerated Requalification Program.

6.1.3.2 Semi-Annual Observation and Evaluation Semi-annually (plus or minus one month), each license holder shall be evaluated in writing on the individual's performance and technical competence during simulated and/or actual normal, abnormal and emergency plant conditions. Attachment 6 should be used for this purpose.

The Shift Supervisor or Assistant Shift Supervisor, shall evaluate all licensed reactor operators assigned to his shif t; the Shift Supervisor shall evaluate the SR0's.

Each Shift Supervisor and Operations Department Backup Licensee shall be evaluated by the Operations Superintendent or designee.

The NOTS shall evaluate all other back-up Licenses.

All written evaluations not performed by the Operations Superintendent shall be

The evaluation must then be forwarded to the NOTS for recording in the Requalification Training Checklist (Attachment 2).

Following any marginal or unsatisfactory performance, the licensee must satisfactorily complete a remedial training program in accordance with TDP-113.

The individual licensee is responsible for completing the remedial program on schedule. Upon completion, the NOTS must review the completed work and signify in writing that the program is complete.

Two unsatisfactory evaluations during the license period, or two consyutive unsatisfactory evaluations, if in different periods, must result in removing the individual from licensed duties.

An Accelerated Requalification Program shall be developed, reviewed by the NOIN and approved by the Operations Superintendent.

The Accelerated Program must be completed satisfactorily prior to l

I resuming licensed duties.

6.2 BACKUP LICENSEES Backup licensees shall complete the requirements of all phases of the Requalification Program, except that some requirements may be waived in accordance with the following:

Waivers are appropriate if the normal duties of backup licensees preclude the need for specific requalification training.

Any requests for waivers must be prepared in writing by the license holder's supervisor, reviewed by the Training Manager, and approved by the Operations Superintendent.

An example of a waiver might be for a licensed engineer who is responsible for Technical Specifications.

This individual should not be required to attend the preplanned lecture on Technical Specifications nor participate in the on-shift discussions concerning Technical Specifications.

All waivers must become part of the individual's training file.

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM Each backup licensee who is assigned to the CR-3 site, owner controlled area, or the Nuclear Operations Training Center, and is not actively performing the functions of an R0 or SR0 as defined below shall spend at least one day every four months on an operating shift performing the functions of a Nuclear Operator or Shift Supervisor or Assistant Shift Supervisor as determined by his license.

The following nuclear positions are considered to be actively performing licensed duties and treet or exceed the above requirement.

6.3 RESTRICTED LICENSEES In the event that Licensed Operators or Senior Licensed Operators are restricted to permit manipulation or direction of specific controls only (such as Fuel Handling). special requalification training requirements will be specified in writing and approved by the NOTM and Plant Manager at that time.

6.4 MAINTENANCE AND REACTIVATION OF LICENSE A licensee who has not been actively performing the functions of an operator or a senior operator for a period of four months or longer shall not perform licensed duties until the Vice President, Nuclear Operations certifies to the NRC that the individual has adequate technical knowledge and understanding of facility operation and administration (10CRF55.31[e]) and the NRC grants approval to resume licensed duties.

(a) Satisfactorily complete one or more tests which cover the material in the preplanned lecture series that was missed during the time of non-participation; (b) Review and/or discuss with a Nuclear Operations Instructor or designated SR0/SS all safety related procedure and plant changes which have occurred during the non-participation period; (c) Spend five days on-shift participating in plant activities at the level of the license held; and (d)

Participate in a Simulator Training Program.

Reactivation of the license must use Attachment 8 of procedure TDP-210, "NRC License Application Process", for documentation purposes.

6.5 LICENSED INSTRUCTOR REQUALIFICATION

Instructors maintaining an active operator license shall attend full Requalification Training as described in this procedure. NRC Certified Instructors (except guest lecturers) shall be enrolled in appropriate Requalification Training Program topics to assure they are cognizant of current operating history, problems and changes to procedures and administrative limitations. Credit will be given for those topics taught by or developed by the Instructor as part of the Licensed Operator Requalification Program or Replacement Operator Training Program.

TDP-209 provides more guidance on the Certified Instructor Requalification Training Program.

Guest lecturers who are recognized experts in their field need not meet the Requalification Training Program requirements.

6.6 REPORTS 6.6.1 Quarterly the Shift Supervisor shall forward to the NOTS, via the Operations Superintendent, the Quarterly Review Records (Attachment

5) for the personnel on his shif t.

Backup Licensees are responsible for forwarding their own forms.

The NOTS should review and determine any deficiencies and/or additional training needed.

his shift.

The Operations Superintendent shall forward all licensee evaluations, including Shift Supervisors, to the NOTS for review, development of remedial training as necessary, and recording.

6.6.3 Semiannually, the NOTS shall submit to the Operations Manager and the Training Manager, a report of requalification training completed, the licensed operators and backup licensees that participated, and a makeup schedule as necessary.

The report will irsclude exam grades and evaluations of each individual's knowledge of topics covered in the lectures and on-shift.

In preparing this report the NOTS shall ensure that no individual's entire lecture series is being done through self-study.

6. 6.'4 Semiannually, the NOTM and Operations Manager will approve the training results. Necessary Accelerated Requalification or special training classes will be identified by the NOTS and approved by the NOTM and Operations Superintendent.

6.6.5 Annually the NOTS will review and report the results of the annual written requalification examination to the Operations Superintendent who will evaluate each licensee and recommend special training classes or removal from shift duties, if necessary.

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM 6.7 LICENSE RENEWAL Prior to the license renewal date, the NOTS will review the licensee's entire training record and evaluate the indicated level of knowledge and report the completion of Attachment 2 to the Training Manager. Based on this evaluation, the Training Manager will make recommendations for license renewal by signing Attachments 7 of TDP-210 or for additional training before license renewal.

If additional training is required to be completed by the renewal date, this notation will be included in the application for license renewals.

TDP-210 must be used for license renewals.

6.8 PROGRAM REVISION CAUTION l

I A change to this approved Licensed Operator Requalification Training Program, by which the scope, time allotted for the program, or frequency in conducting different parts of the program is decreased, shall not be made unless first authorized by the NRC. All changes to the Requalification Training Program must be handled in accordance with N00-05,

" Document Control Program".

6.9 PROGRAM EVALUATION The Licensed Operator Requalification Training Program shall be audited at least annually by the Nuclear General Review Committee in accordance with Technical Specifications Article 6.5.2.9.b.

Additionally, the program must be evaluated in accordance with TDP-108, " Training Program Evaluation".

6.10 INSTRUCTOR QUALIFICATIONS The Instructors who teach or otherwise conduct training for this program shall meet the requirements of TDP-209, " Operations Instructor Qualification Program".

TITLE:

LICENSED OPERATOR REQUALIFICATION TRAINING PROGRAM Those Instructors who teach systems that are unique to a nuclear power plant, integrated response, transients and simulator courses, shall be Senior Reactor Operator certified or licensed. The NOTS may utilize a " subject matter expert" to teach any of these unique topics for which the " expert" has exceptional qualifications. An example of an " expert" would be the Instrumentation and Control Supervisor teaching the Control Rod Drive System or the Integrated Control System. Whenever the NOTS uses an " expert" who is not SR0 certified or licensed, he must do so meeting the requirements of TDP-209.

Systems which are common to all power plants such as Secondary Systems may be taught by any technically competent person who has the necessary Instructor skills.

6.11 EXAMINATION PREPARATION The Annual Requalification Written Examination must be prepared,

administered, and graded in accordance with TDP-106, " Examination Preparation, Administration, and Grading".

The lecture series quizzes need not adhere to TDP-106.

6.12 LESSON PLANS All lesson plans used and materials developed must be completed in accordance with TDP-105, " Lesson Plans Preparation".

The NOTS is responsible for developing lesson plans that will provide feedback of operating experience to trainees.

The NOTS shall review LER's, TAP's, SER's, S0ER's, IE Information Notices and other reports of operating experience at CR-3 and elsewhere in the nuclear industry at least quarterly. Based on this review and input from other FPC Departments as appropiate, the NOTS will determine pertinent information to be incorporated into the Licensed Operator Requalification Program. Lesson Plans in the category Special Training shall be prepared, covering this information in accordance with this procedure unless shift instructional sessions are more appropriate.

The NOTS may assign the responsibility for lesson plan preparation to other members of the Training Department staff.

6.13 SCHEDULING Licensed Operator Requalification Training shall be scheduled on a contir.*ious basis so that all required pre-planned lecture series topics a,e covered at least once every two years and other program aspects are conducted as specified.

The actual scledule of implementation must be developed and published in accordance with TDP-117, " Scheduling and Tracking Training".

A.

Purpose Simulator requalifiestion training is intended to aid the utility's training staff in meeting the operator on-the-job requalification requirements. This program is used to complement and amplify the utility's requalification training program by drawing on B&W expertise in the engineering disciplines and scope of supply.

The program includes a review of recent abnormal occurrences applicable to B&W plant operations and a review of the utiiity's abnormal arid emergency procedures. Requalification training promotes and develops +.he proficiency required to operate the plant in a safe manner during normal and emergency conditions.

Emphasis is placed on recognizing abnormal or potentially dangerous plant conditions, utilizing available indications to analyze transients, and determining the proper corrective course of action through utilization of plant specific procedures. Good watchstanding practice, proper use of procedures and good communication of the shif t organization is also stressed.

Classroom sessions for the Simulator Requalification training may vary according to the specific needs and request of each utility.
