The following information is submitted in response to Inspection Report 82-08 concerning the inspection conducted by Mr.

Crowley of your office on March 2-5 and 16-19,1982. One apparent violation was identified.
VIOLATION 10 CFR 50, Appendix B, Criterion V, as implemented by paragraph 17.2.5 of the Hatch 2 FSAR, requires in part that, activities affecting quality shall be prescribed by documented instructions, procedures, and drawings and shall be accomplished in accordance with these instructions, procedures, and drawings.
Division 4, Section 8, paragraph 8.5.3 of CB&I " Nuclear Quality Assurance Manual for ASME Section III Products",
the applicable procedure for in-process inspection of torus modification welds, requires that the C8&I Welding QA Supervisor assure that joints are properly fitup and aligned prior to welding.
Detail "C" on contract drawing 71, revision 5,
requires that both members of weld B-4 on drawing R56, revision 1 (torus downcomer reinforcement weld), be beveled 150 Contrary to the above, on March 18, 1982, activities affecting quality were not accomplished in accordance with documented procedures and drawings in that C8&I Welding QA Supervisor had signed off the fitup for weld B-4 and one member of the joint was cut square in lieu of the required 150 bevel.
This is a Severity Level V Violation (Supplement I.D.).
RESPONSE
Admission or denial of the alleged violation:
The violation occurred as stated.
Reason for the violation if admitted:
The violation occurred because the degree of bevel was treated as a non-critical prerequisite to the integrity of the weld joint; thus, the degree of bevel was not a checkpoint in the Welding Supervisor's inspection of the weld joint.

A review of the Welding Supervisor's prior inspection was performed with no additional problems encountered.
CB&I reminded their supervisors of their responsibilities in regards to procedure compliance.
Site QC has increased its surveillance of CB&I activities and both site QA and CB&I QA have performed audits of CB&I activities with no significant findings.
Corrective steps which will be taken to avoid further violations:
Increased QC surveillance in future CB&I activities should identify any potential problem areas and allow them to be resolved prior to work being completed.
By means of Nonconformance Reports and the stop work authority (if required), site QC will be able to assure that identified problems are resolved prior to the system being returned to operable status.
Date when full compliance will be achieved:
Full compliance was achieved on March 30, 1982.
