NRC INTEGRATED INSPECTION REPORT 50-272/97-14, 50-311/97-14

NOTICE OF VIOLATION

Dear Mr. Eliason:

On July 26, 1997, the NRC completed an inspection at your Salem 1 & 2 reactor facilities.

The enclosed report presents the results of that inspection.

During the 5-week period covered by this inspection, your conduct of activities at the

Salem facilities was generally characterized by professional and safety-conscious

. operations. Plant and operations management implemented appropriate and timely

corrective actions to address a number of operator performance errors that occurred in the

early to middle part of the inspection period. Recent operator performance has improved.

We are concerned, however, about a violation of NRC requirements that was identified

involving operator procedural adherence. Specifically, the NRC inspectors identified that

on July 2, the operators did not remove test equipment from the 2A emergency diesel

generator as required by a surveillance test procedure.

The inspectors noted that operators are required to enter the Technical Specification action

statement upon opening the control room envelope instrument panel backdoors, and were

concerned about the frequency of and controls established for these entries. Operations

management has since strengthened the administrative controls for entering the panels.

Your staff indicated that modifications were being developed to correct this condition.

The violation is cited in the enclosed Notice of Violation, and the circumstances

surrounding the violation are described in detail in the enclosed report. Please note that

you are required to respond to this letter and should follow the instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part,

to determine. whether further enforcement action is necessary to ensure compliance with

regulatory requirements.
