The licensee, Public Service Electric anct Gas Company (PSE&G}, received a license (DPR-75} on April 18, 1980, which authorized Unit 2 to load fuel and perform low power testing.at power levels not exceeding five percent.

- February 26, 1981 Extended the terminal date for the license for Fuel Loadina and Low Power Testing.

~

On May 20, 1981, the 11cense for fuel loading and low power testing was superceded by Facility Operating License DPR-75.

Facility Operating License DPR-75 authorized PSE&G to operate Salem Unit 2 at 100 percent power.

However, prior to operating at this power level PSE&G was required to complete certain outstanding actions as described below.

Prior io operating above 5 percent power, PSEB"G was required to:

1. Complete the training portion of the Special Lo11J Power Test Program;
2.

Resolve deficiencies in tile SMII-6 Open Items list.

3. Take specific corrective fire ~rotection actions in accordance with an on-site review of the Fire Protection program;
4. Complete additional testing required by IE Bulletin 80-06 and correct reset actions for ti/Jo sets of valves;
  • 5. Train control room operators in the recognition and mitigation of LPI performance degradation;
6.

Submit requested information on Category I Masonary Walls, and

7.

Resolve an issue concerning time available for operator action to prevent au xi 1. i ary feed1r1ater pump damage *.

Items 1 thru s* were verified as completed by the l&E Resident Inspector at Salem 2.

The Salem 2 LPM verified the receipt of the information required by item 6.

By letter dated June 12, 1981 we informed PSE&G that we had reviewed the information submitted ~or item 7 and considered this matter resolved.

Additionally, the following outstanding actions subjected to license conditions have beel',l Gompleted.

By letter dated June 30, 1981, PSE&G reported leak rate measurements for tt10se systems in Hhich leal<age is measured during shutdown.

By letter dated June 30, 1981, PSE&G submitted procedural guidelines for and a description of the RCS vents.

This information was required by July 1, 1981.

PSE&G has reduced its containment setpoint pressure to 4.0 psig.

This action has been verified by the I&E Resident Inspector. A srn..approving this as the minimum setpoint compatible t1ith normal operating conditions has been prepared and approved by CS8; and forwarded to DL.

PSE&G has installed a high radiation isolation signal on the contain-*

ment purge and vent isolation valves.

This action, required by July 1, 1981, \\*Jas verfied by the I&E Resident Inspector.

PSE&G has installed the required containment water level monitoring instrumentation.

By letter dated July 1, 1981, ~1e received a request for Amendment to Facility License DPR-75.

This request is related to license condition 2.C.(25)(e),

11Relief, Safety and Block Valve Test Requirements.

11 PSE&G was required to sub-

-mit a report based on an EPRI generic test program by July l, 1981. The EPRI test program is not complete and hence PSE&G is unable to complete their report.

A license amendment is being prepared to establish a new schedule for this submittal.

By telephone on July 7, 1981 we were alerted to a potential problem related to 1 i cense con di ti on 2 oC ( 10).. PSE&G is required to:

( 1) re vie~"' its fire inter-acti on analysis any correct and additional deficiencies by July 31, 1981; (2) complete final engineering verification of its fire protection analysis by July 15, 1981; and (3) complete all required cable wrapping by July 31, 1981.

PSE&G may be unable to satisfy this schedule and is looking for a two v~eek extension.

They were requested to submit a letter stating the current status of their program, providing justification for an extension, and requesting a license amendment.

By letter dated July 10, 1981 (received July 14, 1981), PSE&G submitted a license change request for this sc~1edu'ie revisfono As a result, /\\rnendment 1 to the Facility Operat*ing License, No. DPR-75, was issued on July 20, 1981.

By 1 etter dated June 16, l 981 PSE&G has requested a 1 i cense amendment to, ct1ange the Technical Specifi cations pertaining to the emergency safety feature~:

actuation instrumentation system, Undervoltage Protection.

This request' -is related to license condition 2.C(15)(4) whi.cn requires PSE&G to :install ca second 1 evel of undervo1 tage protection for the emergency buses by the ind,'_ of the first refueling outage.;

Since tl1e equipment modifications are not installed we notified* PSE&G by letter dated July 9, 1981, that \\*Je have deferred final action on this amendment request until the modifications are complete.

D. The above paragraphs discuss the Salem Unit 2 license conditions which have been the subject of staff activities since the license was issued. The status of the license conditions which have not required staff action since license issuance remains as stated in SER Supplements 4, 5, and 6.

References to the applicable sections of these Supplements are listed in the license for each conditiono By letter dated July 7, 1981, PSE&G requested a ternporary exemption from the Prompt Notification System requirements of 10 CFR 50, Appendix E.

Due to procurement problems PSE&G states it will be unable to complete the fina.l installation until October. 1, 1981.

I&E is currently reviewing this request.

By letter dated March 19, 1981, PSE&G requested four exemptions from III.G. of Appendix R to 10 CFR Part 50.

CEB has completed its review of this submittal and recommends that two of the requests be accepted and that two be denied.

This issue is being resolved jointly between G. Meyer and i1J. Ross as it applies to both Units l and 2. A meeting with PSE&G was conducted on July 23, 1981, to discuss this subject.

PSE&G has committed to providing additional information to support exemptions for specific areas in the plant.

For Salem 2 there are currently three active multi-plant actions.

The status of these items is as follows:

1)

Seismic Qualification of the Auxiliary Feedwater System (TAC 43674) -

Generic letter 81-14 was issued February 10, 1981.

This letter requested that responses be provided within 120 days.

After the

  • deadline had passed with no response it was determined that the utility llad no record of this 1 etter.

Hence, we forwarded an addi ti ona l copy of this 1 etter and extended the response deadline until October 1981.

2)

Station Blackout Procedures and Training {TAC 43949) -

Generic letter 81"."04 was issued February 25, 1981.

This letter requested that an assessment of tl1e capabi 1 i ty of the plant procedures and training program at Salem to mitigate a station blackout event.

By letter dated June 3, 1981, PSE&G requested an extension until September 1, 1981 for submittal of this information.

This extension vms denied by letter dated July 3, 1981, and PSE&G ivas requested to proniptly reply.

3)

Environmental Qualification (TAC 42533) -

14. Safety Evaluat1on for the Environmental qualification of safety related electrical equipment at Salem Unit 2 vJas issued May 28, 1981.

Salem Unit 2 had no equipment which was :identified as rtiquiring immediate corrective action.

The safety evaluation requires PSE&G to supply additional information within 90 days of receipt of the letter.

This response is due in accordance with the operating license on or before August 27, 1981.
