A endix A NOTICE OF VIOLATION American Electric Power Service Corporation Docket No. 50-315 Based on the inspection conducted on July 10, 12, 14," 17, 19-21, 24, 26-28; August 9-10, 14, and 16-18, 1978, it appears that certain of your activities were in noncompliance with NRC require-
- ments, as noted below.
The item is an infraction.
Technical Specification 6.8.1 requires written procedures be established and implemented for equipment control (e.g., locking and tagging; item A.3 of Appendix A Regulatory Guide 1.33, November 1972).
Contrary to the above, on August 12, 1978, the facilities procedure for equipment control, PMP 2110 CPS
.001 "Clearance Permit System,"
was not implemented when the status of a valve on the vent header (WD 244) and which was controlled by striped tag clearance No.
11812 was changed without the permission of the person to whom the clearance had been issued (Shift Operating Engineer) as required by the procedure.
The above caused the unplanned release of the partial contents of two gas decay tanks without prior sampling as required by Appendix B Technical Specification 2.4.4.e.
