Mr. James P. O'Reilly LER No.82-049 Page Two December 17, 1982 DETAILED DESCRIPTION OF EVENT On November 18, 1982, with the Plant in Mode 3, an NRC Inspector notified the Licensee that Technical Specification 3.7.9.1, Surveillance Pequirement 4.7.9.1.1.(e), had not been performed.

The surveillance entails a three (3) year flow test of the Fire Suppression Water System which was required to be performed prior to Mode 6 entry on August 10, 1982.

PROBABLE CONSEQUENCES l

No adverse consequences resulted from this event.

A major flow i

test conducted during a routine ANI survey in June 1982 on the Transformer Deluge Waterspray System adequately demonstrated the full flcw capability of the underground water supply.

CAUSE(S) OF THE OCCURRENCE Personnel assigned the task of tracking the completion of all Surveillance Requirements for Mode 6 entry had previously been advised that Surveillance Requirement 4.7.9.1.1.(d) and (e) were included in Surveillance Test Procedure (STP) 128.020 performed on June 15, 1982.

On August 6, 1982, the Fire Protection Group realized that the three (3) year flow test identified in Surveillance Requirement 4.7.9.1.1.(e) had not been adequately covered in STP-128.020 but were unaware of the restriction on Mode 6 entry.

A new procedure was developed but not performed due to the lack of approved test equipment which had been on order since July 1982.

IMMEDIATE CORRECTIVE ACTIONS TAKEN During the NRC Exit Interview on November 19, 1982, the Station Manager committed to the performance of the missed surveillance test as soon as possible.

The test equipment (cap gauge) which was still on order at time of event was subsequently manuf actured at the plant site, and a satisfactory test was performed at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on November 19, 1982.

Mr. James P. O'Reilly LER No.82-049 Page Three December 17, 1982 ACTION TAKEN TO PREVENT RECURRENCE The following actions were taken by the Licensee:

1.

Personnel responsible for the Fire Protection Surveillance Test Program were counseled regarding the importance of completing the required testing as specified in Technical Specifications.

2.

The Surveillance Test Tracking Program is being updated to include procedure STP-128.021.

This update is expected to be complete by January 15, 1983
