DETAILED DESCRIPTION OF EVENT

On October 8, 1982, with the Plant in Mode 3, Surveillance 4

Test Procedure (STP) 303.006, " Steam Generator "B" Steam

.'.i Pressure (IPT-0484) Instrument Operational Test," was bsin'g

performed.

During the test, it was discovered that a pressure bistable (IPB-0484C) would not change output state despite varying the input about the trip and reset points.

This bistable provides a signal to the "B" loop main feedwater isolation valve logic.

The channel was placed in the tripped position per Technical Specification 3.3.2, (Table 3.3-3) te s.

Action Statement 19.

Investigation revealed that a logic card s

had failed.

The logic card was replaced, and the system was y

returned to operable status per STP-303.006 on October 8, 1982.

PROBABLE CONSEQUENCES There was no adverse consequence as the Action Statement of- ~

the applicable Technical Specification was being complied with.

CAUSE(S) OF THE OCCURRENCE The occurrence is attributed to a failed printed circuit logic card.

IMMEDIATE CORRECTIVE ACTION TAKEN 4

The affected bistable was placed in the tripped position as required by Action Statement 19 of Technical Specification 3.3.2.

The failed card was replaced, and STP-303.006 was performed satisfactorily.

The system was returned to operable status on October 8, 1982.

ACTION TAKEN TO PREVENT RECURRENCE The licensee plans no further action regarding this event since this appears to be an isolated failure.
