| Title | Failure to Maintain a Functional Ammonia Alarm |
| Description | On January 18, 1989, the NRC issued Amendments 61 and 42 to the LaSalle Operating Licenses, NPF-11 and NPF-18, approving the removal of ammonia detectors from the LaSalle Technical Specifications. Letters from C. Allen, Commonwealth Edison [the licensee for LaSalle at the time] to the NRC, dated March 6, 1987, as supplemented on January 6, 1988, March 9, 1988, and January 6, 1989, were incorporated into the amendments. Commonwealth Edison committed to leave the alarm function of the ammonia detectors in place.
Contrary to the above, as of June 20, 2015, LaSalle procedures allowed for the continued reliance on a single operating train of control room ventilation without any functional ammonia detectors. On that date, both detectors associated with the in-service train of control room ventilation were non-functional and would not have provided an alarm to control room staff so that operators could initiate the emergency ventilation filtration system |
