Containment Isolation Valve CFV-16 inoperable contrary to Technical Specification 3.6.3.1.

Conditions Prior to Occurrence:

Mode 3 hot standby.

Description of Occurrence:

At 2250 while performing 0P-401, Core Flooding System, it was discovered that Containment Isolation Valve CFV-lC, Core Flooding Tank 3A Vent to Waste Gas Decay Tank, would not move from mid position. At 0245 on 5 March CFV-16 was isolated and maintenance was initiated. The valve was returned to operability at 0500 on 6 March after satisfactory ccapletion of SP-370, Quarterly Cycling of Valves.

Designation of Apparent Cause:

The causi of this event is attributed to the valve stem binding on the bushing due to insufficient lubrication.

Analysis of Occurrence:

There was no effect upon the health or safety of the general public.

Corrective Action

The valve stem and bushing were replaced and the valve was repacked.

Stroke and leak tests completed satisfactory per surveillance re-quirements. All limitorque isolation valves inside containment have been lubricated and will be placed on the plant lubrication program.

Failure Data:

This' is.the first occurrence of CFV-16 inoperable and the thirty-fourth event. reported under this Specification.
