2.1 PURPOSE AND SCOPE The purpose and scope of this instruction is to supplement Reference 1-A and to provide the inspection criteria and documentation requirements for inspecting Class 1E electrical cable installations in the following:

a.

Unit 2 Safeguard Building b.

Unit 2 Turbine Building 3.0 INSTRUCTION The QC inspector shall verify that an authorized electrical engineer has certified.,by signing the pull card, that the raceway evaluations have been performed.

The QC Inspector shall verify that the cable pull card is in accordance with Reference 1-B.

In some instances the cable pull card will not reflect the route shown in Reference 1-B.

In this event the QC Inspector shall verify, prior to any further processing, that the cable pull card revision code is the same revision code as the latest revision of Reference 1-B, Section ninety-eight (98).

The QC Inspector shall visually inspect the raceway to the maximum extent possible without disassembly, for pulling hazards such as construction debris, rough edges, burrs, etc., which could cause damage to the cable being installed.

NOTE:

It is acceptable to pull cable in cable tray which has thermolag applied without the removal of thermolag.

Quality control verification of tray section has been established prior to installation of thermolag.

Thermolag shall be removed at transition points.

Upon completion of installation, cable installations shall be inspected to verify they are identified, spaced and tied down.

Spacing and tying down does not apply to cables installed and routed beneath thermolag.

l Multiple cable pulls through short lengths of conduit (10' or less) need not be treated as a multiple pull and can be hand l

pulled.

3.1 CABLE INSTALLATION l

3.1.1 Verification of Cable The QC Inspector shall visually inspect the cable which is to be pulled for cuts and nicks in the cable jacket to the maximum extent possible prior to actual cable installation.

AUG 0 21985 The cable jackets color and cable type code shall be in accordance with the cable pull card.

Cable type codes are provided in Attachment 1 of this instruction.

NOTE 1:

Inside containment triax is tefzal insulated, identified by having a slick outer jacket.

(#12240-C-G20).

Outside containment triax is XPLE, identified by having a rubber type outer jacket (#14362-C-G18).

3.1.2 Tension Monitoring Device When it has been determined that a tension monitoring devic 2 is required for a cable pull, Electrical Engineering shall issue a detailed Operation Traveler indicating the tension requirements.

If Electrical Engineering has determined that a cension monitoring device is required for the cable pull, the QC Inspector shall verify the following:

a.

Visually inspect the immediate cable pulling area to verify the absence of cable pulling hazards such as rough concrete, oil, metal shavings or sharp objects which would result in cable damage.

b.

Tension monitoring device (dynamometer) is calibrated and is within the calibration due date specified. Record the M&TE number and calibration due date on inspection report.

c.

A dynamometer in conjunction with mechanical tuggers will be used when pulling Class 1E and Class 1E associated cable (s).

The tension monitoring device will be positioned to allow for a steady continuous pull.

d.

Sheaves are to be placed in such a manner that cables are aligned with tugger.

The radius of the sheave must be larger than the minimum band radius of cable (s) being pulled.

Cable (s) must exit straight from conduit during pull, with the cable on the feeding end of the conduit being hand-fed.

e.

Single conductor power cables (600 V and 8 KV) are attached to a pull rope in the following manner:

1.

12" to 15" of cable insulation and shielding material is removed.

One-half (i) of the bare strands (i from opposite sides of cable) is separated with the remaining strands cut off as close to the insulation as possible. No conductor strands are to be removed when pulling through conduit having no bends.

(See sketch) 3.

The two groups of strands are passed through the eye in the pulling rope or swivel so they cross each other with the strands bent back toward the insulation portion of the cable and formed as tight as possible to minimize the size of the pulling head.

Pulling swivels shall be used when pulling cable through 2" and larger conduit, except where speciffcally noted on pull card by Electrical Engineering.

4.

The exposed conductor strands are to be wrapped with cloth for padding and then taped with duct tape or vinyl tape. The tape shall cover the eye of the rope /

swivel.

Multiple conductor cables are to be accached by stripping approximately 12" of outer and inner insulation and dividing the conductors in half, looping each group of conductors through the rope / swivel eye in opposite directions.

Each conductor is to be attached to the rope / swivel. Wrap conductors as described in Paragraph e.4.

Ke11um's cable grip, Greenlee or approved equal, may be used where power pulling attachments are required.

AUG 0 21985 A QC Inspector shall station him/herself at the tension monitoring device and witness cable tensions, to determine the maximum tension achieved during the pull, when it has been determined by Electrical Engineering that tension monitoring is required. The maximum allowable tension and the actual tension achieved shall be recorded on Attachment 3.

NOTE:

Pulling tensions are not required to be measured when the pull is entirely in cable tray.

These instructions shall be applicable where necessary during cable removal or re-installation requiring tension monitoring.

3.1.3 Swabbing of Conduit The QC Inspector shall witness swabbing activities, and ensure that the actual swab being used is a definite " swab" which encounters the entire cross sectional area of the conduit.

If, during swabbing activities flaking exists, additional swabbing is required.

3.1.4 Cable Pulling During cable pulling operations the QC Inspector shall monitor the raceway through which the cable is being pulled.

Random lay six-hundred (600) vole power cables, cor.crol cables.

instrumentation cables and thermocouple extension wire to be installed in cable tray that has thermolag installed on it may be pulled therein without removal of the thermolag. However, the thermolag shall be removed at cable tray transition points (i.e.

tee's, crosses, elbows, drops, risers) before pulling cable.

Refer to drawing 2323-El-1702, Detail 23A for use of split insulation on tray edge,s at exit points (except for containment).

The Inspections shall includes a.

Minimum Bend Radius - The QC Inspector shall verify that the cable being installed does not exceed the applicable bend radius, Reference Attachment 4.

Areas where minimum bend radius violations are most likely to occur are at points of entry to, and exit from conduit, cable tray and equipment panels.

b.

Cable Damage - The QC Inspector shall verify that the cable does not sustain damage during installations cuts or nicks to jacket

Pull Points - The QC Inspector shall verify that pull points are actually utilized. However,-there are some instances where pull points are not required to be used, the allowable

, /'

exceptions are; cables may not be pulled through any condulets except for Mogul BC type provided the distance between the adjacent pull points does not exceed the dimensions given on the applicable tables' Reference.

Full sleeves and pull boxes shall be used as a pull point except where the distances between adjacent pull points does not exceed the dimensions given on the applicable tables, Reference Attachment 2.

NOTE:

Pulling through condulets other than BC types shall be evaluated and approved on a case-by-case basis by Electrical Engineering.

d.

Pull Boxes - Electrical pull boxes which are unscheduled on the drawing may be installed to minimize conduit installation problems. The size of the field added boxes shall conform and be inspected to the following criteria:

1.

Systems 600V and below Straight Pulls - the length of the box shall not be less than eight (8) times the trade diameter of conduit.

Angle or U-Pulls - The distanci between each raceway entry inside the box and the opposite wall of the box shall not be less than six (6) time the trade diameter of the largest raceway. This distance shall be increased for additional entries by the amount of the sum of the diameters of all other raceway entries on the same wall of the box. The distance between raceway entries enclosing the same conductors shall not be less than six (6) times the trade diameter of the largest raceway.

2.

Systems over 600V and N.I.S.

Pull box size requirements for systems over 600V and N.I.S. will be provided by Electrical Engineering.

e.

Cable $1ack - The QC Inspector shall verify that two (2) inch minimum siset La provided at transition points between cable tray sections at different elevations and at conduit entries to cable trays and between Category 1 structures.

Cable Route - The QC Inspector shall verify the cable is routed in accordance with the latest revision of the cable pull card.

There will be instances during cable pulling operations when the cable enters or exits from cable tray segment (node) numbers which do not correspond to the locations specified on the Cable Pull Card. The following cable card routing exceptions are listed below:

1.

If the cable exits after the node number called for on the Cable Pull Card, ths installation is unacceptable and the Cable Pull Card shall be revised to include additional node numbers.

Revisions shall be authorized by a DCA from Engineering.

2.

There is, usually, more than one node number appearing on the Cable Pull Card at tray fittings such as TEE's and X's.

The Cable Pull Card need not be revised as long as the cable is routed thru at least one of the node numbers specified on it and is being routed in the direction of the the cable route. Otherwise, revisions shall be authorized by a DCA from Electrical Engineering.

NOTE 1:

The acceptable range of any segment (node) number shall include all of the tray between the segment (node) number specified plus one (1) segment (node) number on each side:

Cable Lubricant - The QC Inspector shall verify that the cable lubricant utilized during cable installation, has been approved by Electrical Engineering, and is used on cable that are being pulled into conduit already containing cables.

NOTE:

For Anaconda cable, only Polywater Manufactured by American Polywater Corp. shall be used as a cable lubricant.

h.

Pulling Lines - Hemp or nylon rope, fish tape, and wire rope are approved pulling lines, with the following exceptions:

vire rope shall be used for pulling cables through metal conduit only when no other cables have been previously installed in conduit, and fish tape, wire rope shall not be used for pulling cables in conduits which contain non-metallic bends. A swivel is required for all pulls in two (2) inch conduit or larger unless otherwise noted on pull card or tension calculation sheet.

Exception; swivels are not required when fish tape is used as a pulling line.

1.

Discontinuation of Cable Pull - Cable pulls shall be discontinued for the following reasons:

1.

Damage to the cable before or during the process; cuts in cable jacket which would indicate a protrusion in tray or conduit.

CAUTION: Repaired cable (s) shall not be installed inside conduit.

2.

Exceeding allowable bending radius.

3.

Any deviation from the pull card not authorized by the engineer through a DCA.

3.2 INSPECTION OF PARTIAL PULL The QC Inspector shall inspect and document partial pulls as follows:

a.)

The QC Inspector shall check each inspected attribute (1-11) on Attachment 3.

b.)

The QC Inspector shall document all information contained in Item No. 12 of Attachment 3 upon completion of partial pull activities.

Upon completing inspection of all attributes, the QC Inspector shall sign and date block 10 of Attachement 3.

All "unsat" attributes noted during the initial and subsequent partial pull processes shall be remedied or waivered prior to commencement cf cable pull activities. All waivers shall be annotated in Block No. 14 of.

c.)

Subsequent cable pulls shall be documented on a new IR.

The QC Inspector shall record any previous partial pull inspection report number (s) in block no. 14 of the new Inspection Report and also record the new IR number in Block No. 14 of the previous partial IR.

NOTE:

The cable must be pulled to the equipment to constitute a complete pull, all other conditions that prevent cable installation at the origin or destination end are considered partial pulls.

3.2.1 Partial Pulls Previously Installed - Unit 1 and Unit 2 In order that existing documentation be closed out for cables installed as partial pulls prior to 6/10/85, the QC Inspector shall SAT the attributes not already checked on the existing Inspection Report and close that IR.

Completion of the pull shall be documented on a new IR, Attachment 3, as described in 3.2 above.

3.2.2 Cable Pulls Between Unit I and Unit II When cable pulls include both Unit I (common buildings inside security boundary) and Unit II, the PFG shall provide both a Unit I and a Unit II Inspection Report. Documentation shall be as follows:

a.

That portion of the cable pull in Unic I shall be inspected and documented per Reference 1-4.

b.

That portion of the cable pull in Unit 11 shall be inspected and documented per this instruction.

If no unsatisfactory condition exist the IR shall be closed. A note shall be added to Block #14 which states "This cable pull requires a Unit I/or II Inspection Report to be complete."

3.3 NUCLEAR INSTRUMENTATION SYSTEM CABLE Prior to any Nuclear Instrumentation System (N.I.S.) Triaxial Cable Testing the QC Inspector shall verify that the cable has been in class "A" Storage for a minimum of forty-eight (48) hours.

NOTE:

Class "A" Storage: A fire-resistant, tear resistant, weathertight and well ventilated building or equivalent enclosure which is further equipped for temperature and humidity control and filter air ventilation.

The QC Inspector verify N.I.S triaxial cable testing complete prior to release from the Warehouse.

Acceptance will be in writing, signed by Westinghouse and Electrical Engineering representatives.

3.3.1 Field Inspections of N.I.S.

The QC Inspector shall perform field inspection of N.I.S. cable as follows:

a.

Field Storage - The QC Inspector shall verify that all N.I.S. triaxial cable on reels remaining in the work area shall meet " Class C" storage requirements. Designated areas will be marked off away from traffic and the reels stored in that area.

NOTE:

Class "C" Storage:

A fire resistant, tear resistant, weathertight and well ventilated building or equivalent enclosure which does not require temperature and humidity control.

b.

Cable Installation - The QC Inspector shall monitor N.I.S.

cable installation. All N.I.S. cable shall be handpulled utilizing all designated pull points.

Pull points will be used to assist cable pulling activities only.

NOTE:

The Maximum pulling tension of fifty (50) pounds will not be exceed for individual or multiple pulled cables unless approved by the manufacturer.

Tension Requirements - The QC Inspector shall refer to the criteria in Paragraph 3.1.2 of this instruction.

d.

Pulling Lines - Fish tape may be used as a pull rope without the use of a swivel. Tag Rope will not be used.

e.

Cable Lubrication - The QC Inspector shall verify that conduits have been swabbed and prelubricated prior to installation of the cables using only an electrical engineering approved lubricant.

f.

Cable Training - The QC Inspector shall verify a minimum of two (2) inch slack remains at all transition points (i.e.

junction boxes, entering or leaving conduits).

g.

All cables will be continuous runs between:

1.

Termination at the equipment racks and outboard penetration pigtails / pre-amplifiers, if used.

2.

Inboard penetration pigerils/ pre-amplifier, if used, and N.I.S. detectors.

h.

Cable Bends - The QC Inspector shall verify that all cable bends have an inside radius greater than six (6) inches, or as approved by the cable manufacturer.

NOTE:

Cables will be installed without splices.

i 3.3.2 Pulling Additional N.I.S. Cable When pulling additional cable into conduit containing N.I.S.

cable the QC Inspector shall inspect for the following:

a.

Nylon pull ropes or swivels shall not be used.

b.

A metal fish tape (free from burrs) shall be routed thru the conduit in the reverse direction of the cable pull.

c.

A pre-lubricated swab shall be attached between the fish tape and the cables using a nylon rope.

The distance.

between the end of the fish tape and the cables shall not exceed two (2) feet.

d.

The cables shall be lubricated as they are routed into the conduit.

Upon completion of pulling additional N.I.S. cable into conduit containing cable, the QC Inspector shall immediately initiate a nonconformance report to assure positive tracking and control of required testing.

3.4 SPACING FOR POWER CASLES The QC Inspector shall inspect cable spacing. Maintained spacing between cables shall be measured edge to edge.

The criteria for maintained and nonmaintained cable spacing is as follows:

a.

Where shown as maintained-space cable in the Cable and Raceway Schedule and cable pull cards, on - conductor cable sizes 2/0 AWG and 4/0 AWG run as a 2-wire circuit (e.g., de circuits) or 4-wire circuit (e.g., lighting panel feeders) shall be bundled together aud tied to the cable tray rungs (ladder type tray) with maintained-spacing between each circuit bundle, with the exception of 600 V power and 8kV cable. Maintained spacing shall be twice the diameter of one conductor of the bundled cable.

See the following example for bundling and spacing of cables:

Maintained spacing for 600V and 8kV cable -- Power cables run in cable tray shall have maintained cable spacing where so indicated in the cable and raceway schedule and Cable Pull Cards. Maintained spacing between cable shall have a minimum of one-quarter of the diameter of the largest. cable.

Also, a minimum separation of one-quarter of the cable diameter shall be maintained between side rail of cable tray and adjacent cable.

An example of maintained spaced cable with each three-phase circuit consisting of three (3)

c.

Maintained and non-maintained cable spacing -- In some cases maintained and non-maintained spaced cables will be run in the same tray where indicated in the Cable and Raceway schedule and the Cable Pull Card.

In these instances, non-maintained and maintained spaced cables shall be run on opposite sides of the tray.

"It is not necessary of maintained-spaced cables to conform to these requirements at transition points in the tray system (i.e., where entering or exiting cable tray, in cable tray tees and crossed, etc.).

However, this random lay should not be more than four feet.

3.5 CABLE TIES Cable ties shall have a positive locking device to prevent slippage. Cable tie surfaces in contact with cable jackets shall be relatively smooth with no sharp projections, burrs, etc.,

which could damage the jacket.

The excess length of permanent ties shall be cut off after tying.

Ties to tray rungs shall be placed with the tie portion under the rung (if possible) to facilitate removal of excess length of the tie.

AUG 0 21985 In the reactor building, cable ties shall be Thomas & Betts Tefzed TY-RAP or Engineering approved equal.

In the remaining plant areas outside of the reactor building, cable ties shall be Panduit PAN-TY nylon cable ties or Engineering approved equal.

The spacing of cable tie used in securing cables to cable tray shall be as follows:

a.

Power cables (600V and 8kV) which are routed in tray and require maintained spacing shall be tied to tray rungs with cable ties which are spaced at intervals of not more than six (6) feet in horizontal trays and three (3) feet in vertical trays with additional ties at fittings where necessary to maintain fixed positions.

b.

Random lay of 600V power cables and control cable in tray--Cables which are routed in open rung tray with random lay shall be arranged as neatly as possible and may be grouped and tied in bundles. Such bundles and/or individual cables shall be 1) tied to tray rungs at approximately six (6) feet to eight (8) feet intervals in horizontal trays; and 2) at intervals of not more than four (4) feet in vertical trays.

NOTE:

Random lay 600 volt power cable and control cable to be installed in cable tray that has thermal lag installed on it does not have to be tied to the tray rungs by cable ties.

Cable ties in horizontal, solid bottom tray are not required. With vertical solid bottom tray, use cable ties by drilling 1/4" diameter holes at a minimum spacing of two (2) inches in the tray bottom.

c.

Inacrument crays shall have solid boccom tray, unless otherwise specified on design drawings.

Cable ties are not required for horizontal tray runs; however, it is acceptable to use cable ties at vertical tray fittings by drilling 1/4" diameter holes at minimum spacing of two (2) inches in the tray bottom.

When it is necessary to remove cable ties the QC Inspector shall verify that no damage has occured to the cable jacket or insulation.

3.6 CABLE & RACEWAY IDENTIFICATION

AUG 0 21985 3.6.1 Cable Identification Cable identification shall be by tagging and shall be as shown on the Cable Pull Card. All cables shall be identified at termina-tions in equipment and boxes. Cables entering and leaving a junction box and not terminated in that junction box, are not required to be identified.

l Field color coding of Class 1E or associated Class 1E cables will I

be allowed and approved, in writing, from Engineering on a case-l by-case basis.

If field color coding becomes necessary, Class 1E and associated Class 1E cables shall be color banded (adhesive color tape) at intervals not exceeding five (5) feet, and shall be visible at all openings.

3.6.2 Raceway Identification Raceway alphanumeric a'nd color code identifications are defined and inspected under the provisions of exposed conduit raceway and cable tray raceway inspection instructions.

It is not the intent of this instruction to accomplish a detailed inspection of this activity at this time; however, the QC Inspector shall determine that the raceways through which cables are being installed are sufficiently identified to preclude cable routing errors. The QC Inspector shall also verify the following:

Cable tray and conduit raceways shall be identified within fif teen (15) feet from points of entry to, and exit from rac.eways.

2.

Cable tray and conduit raceways shall be identified on both sides of floor and wall penetrations.

l 3.7 CABLE TRAY AND CABLE REMOVAL Prior to the removal of any cable tray section(s) or class 1E or associated cable (s) from a raceway system, the QC Inspector shall verify the craft has in their possession an Inspected Item Removal Notice (I.I.R.N.).

The QC Inspector shall record the 1.I.R.N. number in the Remarks section (Block 14) of the applicable Inspection Report, Attachment 6 for Cable Removal and for Cable Trsy Removal.

NOTE:

The use of the IIRN form is necessary only when the rework is not identified in some other manner.

For example, rework required by process control sheets such as travelers or NCR's, does not require an I.I.R.N. to be prepared.

AUG 0 21985 3.7.1 Cable Tray Removal The removal of cable tray section(s) for rework shall in addition to the I.I.R.N. for cable tray shall require an I.I.R.N. for cable removal and support as well.

The QC Inspector shall inspect the supporting of the cables, and that they are supported in such a manner that they do not exceed band radius, or are damaged due to construction activities, i.e.,

welding, cutting, painting, movement of material and equipment and documented on Attachment 7.

The reinstallation of cables in the cable tray shall be documented on Attachment 3.

The QC Inspector is required to complace only those attributes required to return the installation to its original configuration prior to the issuance of the I.I.R.N.

NOTE:

The QC Inspector shall for the activity place an asterisk in Block 3 of Attachment 7 and record the applicable cable tray nodes (starting and ending) of the Section(s) removed, in the Remarks section of the Inspection Report.

The re-installation of cable tray section(s) shall be inspected and documented per Reference 1-D.

3.7.2 Cable Removal During cable removal the QC Inspector shall perform the followings a.

Minimum bending radius shall not be exceeded during cable removal activities.

If cable cannot be moved without exceeding this condition or causing possible damage to other cables existing in the raceway, craft shall notify Electrical Engineering for resolution.

NOTE:

At the discretion of the Building Manager /

designes, cable removed from raceway may be scrapped by cutting the cable into approx-imately 24 inch (or less) lengths and placing in a scrap receptacle. The OC Inspector shall annotate

" cable was scrapped" in Block No. 14 of Attachment 6.

Items b through e do not have to be completed if cable is to be scrapped.

Items b and g of shall be marked "N/A".

When cable is removed, visually inspect the full length of the cable for possible jacket or insulation damage, nicks, cuts, etc.

If cable is acceptable, both ends shall be sealed and the cable wound on an empty reel.

In the case of partial cable removal, the QC Inspector shall verify that the coiled cable is not susceptible to damage from ongoing construction activities and record the location of coiled cables in the Remarks section of the Inspection Report Attachment 6.

In the case of cables spared in place, unless directed in e.

writing by Electrical Engineering, the cable will not be coiled. The excess cable shall be cut off and the end remaining in the cable tray shall be sealed with Okinite 35 tape or end caps. The location (node point) where the cable is " spared in place" shall be entered in the Remarks section of the Inspection Report Attachment 6.

3.8 SEPARATION The QC Inspector shall perform field inspection of cable pulls for separation per Reference 1-E for air drops only.

3.9 MEGGERING OF POWER CABLES The maggering of power cables shall be inspected in accordance with Reference 1-F.

3.10 INSPECTION OF CABLE GRIPS The QC Inspector shall perform field inspection of cable grip installations per Reference 1-G.

3.11 REVISIONS TO CABLE CARDS Revisions to cable cards when the new card set is received with changes which do not affect the actual installation, such as a new card showing a change in system number, will have the information from the old card transferred to the new card and the revision number of the old cards noted on the new card by craft.

This information is to be reviewed for accuracy by QC and this review documented on Attachment 8 for filing in vault. The new set of cards are now handled as completed, the old card set may be destroyed.
