| Description | 10 CFR Part 50, Appendix B, Criterion III, Design Control, requires, in part, that design control measures shall be provided for verifying or checking the adequacy of the design. Contrary to the above, from March 28, 2000, to September 13, 2012, the licensees design control measures failed to verify the adequacy of installing thermal relief valves on the MSIV accumulators to address issues associated with pressure-related alarms. Specifically, on September 6, 2012, the licensee identified that the MSIV accumulators could experience a higher temperature than previously considered. Changes in ambient temperatures from a HELB in the area will cause pressure to rise and actuate a small relief valve which bleeds oil back to the MSIV oil reservoir. If too much oil is relieved to the reservoir, there may not be sufficient oil remaining to fully close the MSIV regardless of the accumulator pressure. The licensee performed an operability evaluation and concluded that the thermal relief valves must be isolated to prevent a loss of fluid during these conditions, and instituted a compensatory action that isolated the relief valves. At the time of the exit meeting, the licensee had not determined how compliance would be restored long-term. The licensee entered this issue into their CAP as IR 1409900. This issue was determined to be of very low safety significance (Green) based upon a detailed risk evaluation performed by an NRC SRA. |
