[Note - The information in this part of the Summary is often preliminaryin nature and is provided to help IOEB staff communicate and track noteworthy items being followed up by either the Regions or HQ staff.]
- 1) EN 45839 - Part 21 - FAULTY MODULATION INDEX CONTROL CIRCUIT BOARD USED IN A POWER SUPPLY As part of the dedication process the vendor discovered the issue with the two resistors having one leg each
,erminated in the wrong location. Subsequent repair and testing confirmed this to be the issue and that once corrected, the irregularities were gone. The licensee is evaluating all other circuit board that are supplied by 2
this vendor. The residents are aware of this issue and are monitoring the situation. (Forward to TRG for Quality Assurance and Vendor Issues - Paul Prescott - Assigned to Mike Brown).
NOTIFICATION EN 45840 - PILGRIM - LOSS OF EMERGENCY RESPONSE ORGANIZATION IN-PLANT SYSTEM Yesterday, the licensee determined that the Computerized Automated Notification System (CANS) that the event was and back-up system (BEEPS) were not working as designed. The licensee determined was corrected at 0845 caused by a USA-Mobility transmitter failure near the site that serves Pilgrim. The issue toTRG this morning and they have restored full ERO notification capability. (Recommend Screen out, forward fnr Fmprnencv Plannina: Eric Schrader - anedtoMike Brown)
Adjacent to Auxiliary Building (4/12) - Update team to look
- 10) Kewaunee - Tritium in Monitoring Well yesterday. Licensee has formed an investigation building.
Region had a conference call with the licensee the auxiliary
- 707, which is located about 8 feet west of into the source of tritium in monitoring well 1,500 to 2,000 detectable levels, now they are routinely seeing Previous samples of this well were all below their investigation, the They have no known sources of leakage at this time. However, as part of pCi/liter. long), which.
an open container with demineralized water in the aux building (unknown for how licensee left detectable Licensee has confirmed that they do not have wound up with about 400,000 pCi/I of tritium. the NEI guidance. Region will be activity in the auxiliary building. The licensee program is following airborne issues LSteve call with the licensee on April 20. (_Pass to NRR Staff Following Tritium having another phone Bob Bernardo).

- 12) LER 2552010001R0 - Palisades: Potential Loss of Safety Function Due to Service Water Pump Shaft Coupling Failure. This LER reports an event where a SWS pump shaft coupling was found broken.
During the period it was potentially broken, the other SWS pump was out of service for a short period. This LER also meets the reporting requirements of 10 CFR Part 21. The LER provides additional information to a condition which was already reported by Hydroaire under Part 21 number 2009-20-00. (IOEB will update a previous OpE COMM on SW shaft coupling failures.
Recommend pass to TRG Leads for Quality Assurance and Vendor Issues (Paul Prescott) and for Pump and Valve Performance (Russell Lake). Post this LER to the public website as Part 21 Report number 2010 00. Assigned to Steve Pannier - Complete.)
- 13) LER 3692010001R0 - McGuire 1: Power reduction of two units due to entry into LCO 3.0.3 caused by the inoperability of both trains of the Control Room Area Chilled Water System. This LER reports an event where one control room chiller was down for maintenance and the other chiller failed. (Recommend pass to TRG Lead for Control Room Habitability and HVAC (Nageswara Karipineni). Assigned to Steve Pannier).
- 14) LER 4832010001R0 - Callaway: Emergency Core Cooling System MODE 4 Operating Practices Prohibited by current Technical Specification 3.5.3. This LER reports an event where the LOCA procedure was found deficient and could result in the loss of an RHR safety function following a LOCA while in MODE 4.
(Recommend pass to TRG Leads for ECCS (Samuel Miranda), Human Performance (Michael Boggi), and Safety Culture (Alexander Sapountzis). Assigned to Steve Pannier).
,5) LER 2772010001R0 - Peach Bottom 2: Multiple Slow Control Rods Results in Condition Prohibited by Technical Specifications. (Recommend adding this LER information to existina OpE COMM describing this event. Assigned to Issue for Resolution Manager-Mike Brown).
- 16) A NEW INFORMATION NOTICE WAS ISSUED / POSTED TO THE PUBLIC WEBPAGE TODAY IN 2010-09 04/14/20 Importance of Understanding Circuit Breaker Control Panel Indications The recent event at Robinson for which a SIT has commenced (soon'be upgraded to an AIT) has resulted in the quick issuance of this Information Notice. It discusses the Robinson event and some other breaker control power operating experience that has determined that the following items are important to ensure proper breaker operation:
- the verification of control power indication for both safety-related and non-safety related circuit breakers during walkdowns
- the potential impact on safety-related equipment resulting from a loss of contro! power to non-safety related circuit breakers when they fail to open to isolate a fault.
See IN (ML101020184) for full details. The item has also been posted on the NRC public Generic Communication webpage for INs -
(Send IN to the four OpE regional contacts, assigned to Mark King - completed).
