1) EN 45839 - Part 21 - FAULTY MODULATION INDEX CONTROL CIRCUIT BOARD USED IN A POWER SUPPLY As part of the dedication process the vendor discovered the issue with the two resistors having one leg each

,erminated in the wrong location. Subsequent repair and testing confirmed this to be the issue and that once corrected, the irregularities were gone. The licensee is evaluating all other circuit board that are supplied by 2

NOTIFICATION EN 45840 - PILGRIM - LOSS OF EMERGENCY RESPONSE ORGANIZATION IN-PLANT SYSTEM Yesterday, the licensee determined that the Computerized Automated Notification System (CANS) that the event was and back-up system (BEEPS) were not working as designed. The licensee determined was corrected at 0845 caused by a USA-Mobility transmitter failure near the site that serves Pilgrim. 12) LER 2552010001R0 - Palisades: Potential Loss of Safety Function Due to Service Water Pump Shaft Coupling Failure. This LER reports an event where a SWS pump shaft coupling was found broken.

During the period it was potentially broken, the other SWS pump was out of service for a short period. This LER also meets the reporting requirements of 10 CFR Part 21. The LER provides additional information to a condition which was already reported by Hydroaire under Part 21 number 2009-20-00. (IOEB will update a previous OpE COMM on SW shaft coupling failures.

13) LER 3692010001R0 - McGuire 1: Power reduction of two units due to entry into LCO 3.0.3 caused by the inoperability of both trains of the Control Room Area Chilled Water System. This LER reports an event where one control room chiller was down for maintenance and the other chiller failed. 14) LER 4832010001R0 - Callaway: Emergency Core Cooling System MODE 4 Operating Practices Prohibited by current Technical Specification 3.5.3. This LER reports an event where the LOCA procedure was found deficient and could result in the loss of an RHR safety function following a LOCA while in MODE 4.

It discusses the Robinson event and some other breaker control power operating experience that has determined that the following items are important to ensure proper breaker operation:

- the potential impact on safety-related equipment resulting from a loss of contro! power to non-safety related circuit breakers when they fail to open to isolate a fault.
