NRC RESIDENT INSPECTION REPORT NO. 50-254:265/97008(DRP) AND
NOTICE OF VIOLATION
Dear Mr. Kraf t:
This refers to the inspection conducted from May 6 through June 16,1997, at the Quad
Cities Nuclear Power Station. The purpose of the inspection was to oetermine whether
activities authorized by the license were conducted safely and in accordance with NRC
requirements. During tl'is peried routine resident inspections were performed. Based on
the inspectors' findings, overall operations at the Quad Cities station were conducted in a
safe manner. At the conclusion of the inspection, the findings were discussed with those
members of your staff identified in the enclosed report.
During this inspection period, major activities such as the Unit 2 refueling and startup were
performed in a controlled manner and essentially error free. However, less significant
evolutions were not always planned as carefully and occasionally resulted in unexpected
events or challenges to operators. Maintenance workers performed tasks safely and
according to procedures. The successful repair of the reactor core isolation cooling (RCIC)
system steam supply valve was well planned.
The inspectors identified two violations of regulations. The first violation involved two
examples of inadequate procedures used for surveillance testing and emergency operation.
The second violation is a concern because inadequate corrective action resulted in both fire
pumps'becoming inoperable in 1996 due to zebra mussel growth in the intake structure.
These violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detailin the enclosed report. Please note that
;you are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part,
to determine whether further enforcement action is necessary to ensure compliance with
regulatory requirements,
