Technical Specification 3.7.A.2 states in part: " Primary Containment integrity shall be maintained a" all times when the reactor is critical..."

Contrary to the above, primary containment integrity was violated on June 12, 1979, when, with the reactor critical at full power, the torus was opened to the reactor building atmosphere by disassembling the Core Sp.ay A Full Flow Test Check Valve (No. 1400-35).

Although Procedural and Management Controls in effect at the time should have prevented this occurence, it was concluded that these controls were vague. Therefore, to prevent a recurrence, station procedure 1.5.3 has been revised. This revision provides more specific direction for respon-sibile management personnel to ensure that adequate reviews, to prevent violations of limiting conditions of operation, are conducted prior to approval for maintenance activities to commence. A station organizational change which involved the creation of the position entitled, Assistant Station Manager, has served to further enhance overall Station management controls.
