Specifically, the licensee failed to properly lubricate valve 2-8000A because procedure MSE-P0-8349, Limitorque Actuator Periodic Electrical and Mechanical Inspection, Revision 9, failed to require lubrication of the portion of the stem in contact with the operator. The licensee documented the condition in Condition Report CR-2011-004136.
