This report is due within 90 days after the end of the Unit 2 April 2006 refueling outage, which is July 16, 2006. This report documents examinations performed during the first period of the fourth inspection interval.

Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report addresses examinations performed between the end of the previous refueling outage March 28, 2004 through the end of the most recent refueling outage. This most recent refueling outage was the 1 8 'h refueling outage for Quad Cities Nuclear Power Station Unit Two. The outage started March 24, 2006 and was completed April 18, 2006 when the unit returned to service.

This refuieling outage was the second outage during the first inspection period of the fourth inspection interval, which began on March 10, 2003. The first inspection period of the fourth inspection interval ends with this refueling outage on April 18, 2006. This report covers examinations performed to the first period, fourth interval requirements.

Tests, examinations, repairs and/or replacements associated with American Society of Mechanical Engineers (ASME) Code,Section XI were witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports.

The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection & Insurance Company of Connecticut (HS B-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road Suite 500, Lisle, Illinois, 60532-987 1.

1.2.

Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1995 Edition through 1996 Addenda were implemented for Class 1, 2 and 3 components except where written relief has been granted by the NRC. Written relief, in the form of Relief Request 14R-02 approving Risk Informed Inservice Inspection (RI-ISI) as an inspection program alternative for Code Categories B-F, B-J, C-F7-i and C-F7-2, was utilized during Q2R 18. The ultrasonic examination volumes for RI-ISI components were increased, from the Section XI requirements, to meet the RI-ISI Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix VIII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a, "Codes and Standards", and amended by the Federal Register Notice 64 FR 5 1370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CFR 50.55a(g)(6)(ii)(C),

for wrought austenitic piping welds, ferritic piping welds, bolts/studs, reactor vessel welds, nozzle to vessel welds, nozzle inside radius sections and full structural overlaid wrought austenitic piping welds.

Section X1 Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), magnetic particle (MT) and visual (VT) testing were performed prior to and during the 18 1h refueling outage on ISI Program components. Examinations were performed by General Electric (GE), IHI Southwest Technologies (lSwT), and Exelon Nuclear personnel. Volumetric and surface examinations were compared with the recorded results of previous examinations for applicable Class 1, 2 and RI-ISI components. ISI inspections comprised of 35 volumetric (16 were RI-ISI, Category R-A welds), 4 surface examinations and 24 visual examinations were performed on Class 1, 2 and RI-IS! components. Thirteen of the visual exams were subsequent exams due to IWF-2200(b) for corrective action from Q2R 17. Code cases used for IST or Repair/Replacement activities are listed in the ISI Program Plan.

The 13 subsequent exams as required by IWF-2200(b) were a result of the "As-Found" cold settings being out of tolerance for both Constant Spring Can Component Supports and Flued Head Anchors that did not meet acceptance criteria during Q2R17. The subsequent examinations found no additional relevant conditions.

Table 3.1-4A, "Quad Cities Unit 2 ASME Section XI Limited Coverage Summary" contains the volumetric examinations that did not meet coverage requirements of ASME Section XI Code (i.e. greater than 90 percent of the specified examination volume). These exams were limited by design specific geometry. Accessibility could not be improved.

Four of the exams were on nozzle welds that have received relief for exam limitations in the past. The four nozzle welds coverage increased over the previous interval exams. The last limited exam was for exam category R-A where previous single sided access coverage had achieved 100% when including only 1/4" of base metal. Risk informed inspection requirements increased base metal to 1/22" causing this single sided exam coverage to decrease in total examinable volume. Relief requests as required by 10 CFR 50.55a(g)5(iii) and 50.55a(g)5(iv) will be generated and submitted within twelve (12) months following the end of the inspection interval.

A portion of the reactor vessel vertical welds were examined during Q2R17 in accordance with the requirements of I OCFR5O.55a "Codes and Standards" regarding the section entitled "Augmented Examination of Reactor Vessel". The remaining reactor vessel vertical welds were examined during Q2R 18.

Pressure testing VT-2 examinations not related to repair/replacement activities are documented in Table 3.1-2, "Quad Cities Unit 2 Pressure Testing Summary". The Class 1 Vessel Leakage Test and the Reactor Vessel Head Flange Seal Leak Detection System Test were performed in accordance with the 1995 Edition 1996 Addenda of the ASME Code for the first period of the fourth Interval.

Exelon personnel performed functional tests on a total of fifteen (15) snubbers for operability per the Service Life Monitoring/Failure Trending Sample and ASME OM Code 6 of 19

Nine (9) of the fifteen (15) snubbers were ASME Section XI Code designated. Of the fifteen (15) snubbers tested, one was found unacceptable for continued operation (IR# 471469). An additional nine (9) snubbers were tested for expanded sampling due to snubber failure. Eight (8) of the nine (9) expanded sample snubbers were ASME Section XI Code designated. Of the additional nine (9) snubbers tested, two (2) snubbers were found unacceptable for continued operation (IR# 472956 and 472989). The three (3) unacceptable snubbers were replaced with like-for-like units. Snubber testing activities are summarized in Table 3.1-3, "Quad Cities Unit 2 Snubber Functional Tests".

Table 3.1-4 "Quad Cities Unit 2 Conditions Noted and Corrective Actions" summarizes the conditions noted during the examinations and corrective measures taken prior to returning the unit to service.

Table 3.3 identifies number and percentage of examinations completed in the fourth interval.

NRC performed an audit of the Inservice Inspection (IS 1) Program during Q2R1 8 (IR 05000254(265)/2006004). No findings were noted.

3.2. BWRVIP-75/Generic Letter 88-01 Augmented examinations required by Generic Letter 88-01, "NRC Position on Intergranular Stress'Corrosion Cracking (IGSCC) in BWR Austentic Stainless Steel Piping" and BWRVIP-75, "BWR Vessel and Internals Project Technical Bases for Revisions to Generic Letter 88-01 Inspection Schedules", were implemented in the previous outage Q2RI 7. No exams were performed this outage.

3.3. Invessel Inspections (IVI)

To implement the requirements of the Boiling Water Reactor Vessel Internials Program, (BWRVIP), GE was contracted to perform the In-Vessel Visual Inspections (IVVI). The following components and assemblies were examined: Jet Pump Assemblies in accordance with the BWR Jet Pump Assembly Inspection and Flaw Evaluations Guidelines (BWRVIP-41 -A), Core Spray Internal Piping in accordance with the BWR Core Spray Internals Inspection and Flaw Evaluation Guidelines (BWRVIP-1 8-A), Core Shroud Ring Segment Vertical Welds in accordance with the BWR Core Shroud Inspection and Flaw Inspection Guideline (BWRVIP-76), Reactor Pressure Vessel attachments associated with the Feedwater Spargers and steam dryer wall support lugs in accordance with the BWR Vessel ID Weld Inspection and Flaw Evaluation Guidelines (BWRVIP-48-A), and Control Rod Guide Tubes in accordance with BWR Vessel and Internals Project Boiling Water Reactor Lower Plenum Inspection and Flaw Evaluation Guidelines (BWRVIP-47-A).

A 1 0-year re-inspection of the core shroud repair hardware (tie rods) was performed during Q2Rl 8. Additional detailed inspections were performed in accordance with GE and BWRVIP recommendations in response to recent findings at Plant Hatch. No recordable indications wvere identified.

These indications do not pose a challenge to the structural integrity of the core support ring since the shroud stabilizers (tie rods) structurally reinforce welds HI through H7. Another indication was found on the top surface of the Shroud Head Flange ring (IR 475332). The indication appeared to be a scratch or small gouge. It did not line up with any probable heat affected zone and did not have any depth when viewed from the vertical surface.

During the IVVI examinations four (4) jet Pumps were identified with evidence of main wedge (WVD-1) movement. GE Indication Notification Reports (INRs) and Exelon Issue Reports (IRs) were generated to document these conditions (IR 474814, IR 475009, IR 474491 and IR 474070). Additional inspections were performed as required by BWRVIP-41 -A including supplemental visual inspections of the affected jet pumps to confirm overall pump integrity. The most severe wedge movement (on Jet Pump #07) was corrected via a planned restrainer bracket replacement under WO 823272-01. Quad Cities shall continue to monitor the condition of jet pump wedges during future IVVI visual inspections.

Seven of eight Feedwater Sparger End Bracket pins were welded into place during Q2R 18.

This was a planned activity in response to a missing nut on a Feedwater Sparger End Bracket pin identified during Q2R 17, which was welded into place during Q2R 17. In addition, two of the pinheads were observed to be wearing into the bracket hardware during Q2R18 due to flow-induced vibration (IR 474497). This is not a concern for the immediate future. The worst worn bracket still has greater than 90% of its original thickness remaining. Quad Cities will continue to monitor this condition. In the meantime, long-term repair options are being evaluated.

_HCU Vlv Flange leaks Reference EC 359753 evaluating Leakage Observed Report VT2-0 18 Visual Inspections As a result of inspections in accordance with ASME Section Xl IWVE 1992 Edition, 1992 Containment Penetrations XlI and Addenda degraded moisture barriers on penetrations XlI and X2 were identified. XlI and X2 X2 Moisture Barriers were repaired under Work Order ft 907825.

_________________________In Vessel Inspections Core Shroud Ring Segments Indications were observed on two of the core shroud rings during Q2R 18. At the Core Support Flange Ring, multiple crack-like indications were observed at the heat affected zone of circumferential weld H-5 (ER 475862). These indications do not pose a challenge to the structural integrity of the core support ring since the shroud stabilizers (tie rods) structurally reinforce welds H I through 1-17. Another indication was found on the top surface of the Shroud Head Flange ring (IR 475332). The indication appeared to be a scratch or small gouge. It did not line uip with any probable heat affected zone and did not have any depth when viewed from the vertical surface.

let Pump WD-I Vedge Wear During the IVV I visual examinations four (4) jet pumps were identified wvith evidence of main wedge (WD-l1) movement. GE Indication Notification Reports (INRs) and Exelon Issue Reports (IRs) were generated to document these conditions (IR 474814, IR 475009, IR 474491 and IR 474070). Additional inspections were performed as required by BWRVIP-41 a including supplemental visual inspections of the affected jet pumps to confirm overall pump integrity. The most severe wedge movement (on Jet Pump #07) was corrected via a planned restrainer bracket replacement under WO 823272-01. Quad Cities shall continue to monitor the condition of jet pump wedges during future IVV I visual inspections.

Feedwater Sparger End Brackets Seven of eight Feedwater Sparger End Bracket pins were welded into place dturing Q211 18.

This was a planned activity in response to a missing nut on a Feedwater Sparger End Bracket pin identif ied during Q2R 17. In addition, two of the pinheads were observed to be wearing into the bracket hardware during Q2R 18 due to flow-induced vibration (IR 474497). This is not a concern for the immediate future. The worst worn bracket still has greater than 90% of its original thickness remaining. Quad Cities will continue to monitor this condition. In the meantime, a long-term repair options are being evaluated.

Engineering Evaluation EC 360359 determined that all of the indications are acceptable for at least one cycle of continued operation.
