RESEARCH INFORMATION LETTER (RIL 2025-02) A HUMAN ERROR HANDBOOK AND VERIFICATION OF IDHEAS-DATA (INTEGRATED HUMAN EVENT ANALYSIS SYSTEM - HUMAN RELIABILITY DATA)
Enclosed for your information and use is the Regulatory Information Letter, A Human Error Handbook and Verification of IDHEAS-DATA (Integrated Human Event Analysis System -
Human Reliability Data). This report was developed by the staff in the Office of Nuclear Regulatory Research (RES) with contractors from Pacific Northwest National Laboratory (PNNL). The Human Error Handbook is a supplement to the Integrated Human Event Analysis System for Event and Condition Analysis (IDHEAS-ECA) method. The document helps human reliability analysis (HRA) analysts understand and model human events in IDHEAS-ECA.
Examples in the data source information in the Handbook help analysts to relate the guidance to the context of their analyses, thereby improving consistency of applying IDHEAS-ECA to probabilistic rick assessment/HRA applications SRM-090204B directed the staff to develop a human reliability data basis to support HRA method improvement. The staff developed a human error database, referred to as The Integrated Human Event Analysis System - Human Reliability Database (IDHEAS-DATA), in which human performance and error data are identified through extensive data collection.
Subsequently, RES contracted PNNL to review the data sources and verify the entries in IDHEAS-DATA. The staff and contractors also documented the human error data sources using the IDHEAS structure. The documentation about each data source includes the context, tasks, human errors, uncertainties, and the representation of the human error context in IDHEAS-ECA cognitive failure modes and performance influencing factors. The staff compiled the documentation of the data sources into a human error handbook. This report presents the summary of the data verification and the Human Error Handbook.
