Turkey Point Nuclear Plant, Unit 4 L-2024-007 Docket Nos. 50-251 Enclosure Page 2 of 5 TABLE 1 ITEMS WITH FLAWS OR RELEVANT CONDITIONS THAT REQUIRED EVALUATION FOR CONTINUED SERVICE Examination Category and Item Number Item Description Evaluation Description E-A E1.30 Moisture Barrier Degradation AR02421997, AR02422598 and EC297448 Rev2 Discovery: Several areas were identified damaged on the moisture barrier located on the 14 elevation. During the repair phase of the moisture barrier, water was observed to be flowing out of the containment moisture barrier and pooling on the 14-foot elevation of the containment building. Chemical analysis confirmed that water in the moisture barrier was borated water. The source of the borated water was either from the refueling cavity or the spent fuel pool (SFP). The water was pumped from the moisture barrier after cavity drained down.

Conclusion:==

An engineering analysis requested the entire moisture barrier to be removed, washed, and left removed to allow the joint to dry during the current run cycle. The moisture barrier was to be replaced during PTN 4-34, but during PT4-34, the refueling cavity liner in the lower portion of the refueling cavity was sealed with InstaCotetmML to determine if the liner in that section of the cavity was the source of the leakage. Engineering inspection during the outage verified that the moisture barrier area remained dry during PT4-34 confirming that the leakage

Repairs of the lower cavity liner are planned to occur during PT4-35. EC297448 was revised to justify leaving the moisture barrier out for an additional cycle until repairs of the lower cavity complete. This will allow full confirmation that the refueling cavity repairs are effective in eliminating future borated water intrusion into inaccessible areas.
