LICENSEE:
The inspection was an examination of the activities conducted under your license as they relate to radiation safety and to compliance with the
Nuclear Regulatory Commission (NRC) rules and regulations and the conditions of your license. The inspection consisted of selective
examinations of procedures and representative records, interviews with personnel, and observations by the inspector. The inspection findings
are as follows:
1. Based on the inspection findings, no violations were identified.
2. Previous violation(s) closed.
3. During this inspection, certain of your activities, as described below and/or attached, were in violation of NRC requirements, and
were assessed at Severity Level IV, in accordance with the NRC Enforcement Policy.
B. The following violation(s) is/are being cited in accordance with NRC Enforcement Policy. This form is a NOTICE OF VIOLATION,
which may be subject to posting in accordance with 10 CFR 19.11.

17. Scope and Observations:
This was an assist inspection for Region III. The last routine inspection in 2018 identified 2 SLIV violations, for failure
to perform leak tests at least once every three years, and failing to perform annual audits each year. Corrective
actions for these violations were reviewed during this inspection. Cadillac Castings possessed one fixed gauge, a
Thermo Process Instruments 5201, for monitoring the level of raw material going into a melting furnace.
The inspectors interviewed the RSO and observed the installed gauge in use. Area postings were adequate,
independent surveys indicated the gauge shielding was intact as area dose rates were minimal. The inspectors
observed the RSO properly collect samples for leak test analysis and perform a shutter test. The inspectors
reviewed records of previous leak test results, which had been completed as required since the last inspection. The
RSO had a work order generated every six months to ensure physical inventory and shutter tests were performed as
required. The RSO also performed audits of the program each time the work order was generated, and wrote notes
on the work order form to document the audit. Both violations from 2018 are considered closed.
The inspectors noted that the licensee's fixed gauge had been exchanged by the manufacturer in 2011 with a
generally licensed device (s/n B5414) that is still listed in GLTS under recipient code GL-60803. The licensee has
kept this gauge under their specific license and covered the GL label as permitted under 10 CFR 31.5(c)(8)(iii)(B).
However, the licensee is considering terminating the specific license and reverting to possessing the gauge under a
general license as permitted by 10 CFR 31.5(c)(9), and would make the required report if such a transfer occurred.
The RSO stated that they would plan to coordinate any potential future efforts with Region III licensing staff.
No violations of NRC requirements were identified as a result of this inspection.
