Interim Notification per 10 CFR Part 21 LAKE ENGINEERING CO.

Box296 10 Austin Avenue Greenville, RI 02828 Tel: 401-949-4733 Fax: 401-949-0220 December 28, 2021 Degraded Snubber SF1154 Hydraulic Fluid Batch No. 18CLVS431 Between September 25, 2018 and November 18, 2018 Lake Engineering Company (LEC) received from Momentive Performance Materials a total of two 55-gallon drums containing SF1154 fluid from the subject batch. Lake Engineering shipped fluid from these drums to various nuclear plants in 5-gallon containers.
On October 28, 2021 Lake Engineering Company was advised by South Texas Project that amorphous grey matter was observed in fluid from the subject batch that was shipped to STP on April 9, 2019.
From the description provided it was assumed by Lake Engineering that the material observed is a cyclic siloxane byproduct that had been previously identified by Lake Engineering Company in other batches of SF1154.
Within 60 days following the date of this notification, Lake Engineering Company shall submit either a final report or a second interim report documenting the results to date of our evaluation to determine whether the anomaly associated with Batch 17BLVS293 represents a defect which could create a substantial safety hazard at any of the US nuclear plants to which it was provided by Lake Engineering Company.
