Subject:

Control Room Design Review In the referenced letter, which transmitted the Control Room Design Review Report, we stated that a progress report on the implementation of the control room enhancements would be submitted to the NRC within two months following completion of the next refueling outage for Peach Bottom Units 2 and 3.

This letter provides the status of the control room enhancements as of the end of the 7th refueling outage for Unit 2.

A separate letter will be submitted after completion of the 7th refueling outage for Unit 3.

In the Control Room Design Review (CRDR) report, 262 Human

Engineering Discrepancies (HED) were identified.

Each HED was assigned a priority (some separated into two priorities) with respect to safety using a significance scale of 1 to 6, indicating the overall likelihood of the HED to cause operator error.

The HED priority categories and numbers of HEDs assigned to each were:

Not A Discrepancy 39 The subject, schedule, and completion status of the individual Priority 1 and 2 HEDs are provided in Tables 1 and 2 of to this letter.

In summary, the corrective actions for the Priority 1 HEDs were completed during the 7th refueling outage for Unit 2 except for HED El-02, PA System Access, which will be corrected during the 8th refueling outage as originally scheduled.

The corrective actions for the Priority 2 HEDs were also completed during the 7th refueling outage with the exception of HEDs TAl-01, TAl-06, TAl-16, VW-12, SP1-02, and D5-09, which will be completed during the 8th refueling outage or as indicated in Note 1 of Table 2.

The corrective actions for six Priority 2 HEDs has been revised.

The HED Assessment revisions are provided in Attachment 2.

During the implementation of the Priority 1 and 2 HEDs, the corrective actions for many Priority 3 and 4 HEDs were completed.

The completed Priority 3 and 4 HEDs are listed in Table 3 of.

These are presently scheduled for completion in late 1989-early 1990.

Revision of T200 series procedures is in progress. Five (S) have been approved by FORC and issued.

The remainder are in various stages of revision and are scheduled for completion by the end of 1989 Note 2: Controllers were relabeled during the 7th refueling outage. Review shows that some additional labels would be helpful. Application of these labels is not outage related and is forecast complete by 12/31/89.

Ne special cue is available to an operator (e.g a slower blink rate) to indicate when an alarm input clears. The operator must determine,on his own, that the alarm has cleared.

The previous resolution of this HED required the position on of the internal annunciator slide switch to be engraved on the annunciator window. Alarm cards were to be revised to indicate the required setting. Guidelines for setting switches were to be provided. This resolution is revised. The slide switch position will not be engraved on the annunciator windows. Revised annunciator procedures have been prepared to show the required position of the slida switches.

TRAINING REQUIREMENTS:

Train operators in system for required settings.

PROCEDURE REQUIREMENTS:

Revised annunciator window setting procedure now in place REASON FOR REVISION:
