Operating License, NPF-60, Appendix A, Technical Specification.3.S.3 Action C.

l This report addresses a Train A Safety Injection on Unit 2 during a performance of a Solid State Protection System Bi-Monthly surveillance.

On August 31, 1987, while in Hot Standby, Unit 2 shift personnel were performing a Bi-Monthly Surveillance of the Solid State Protection System.

The Train A Output Cabinet Mode Selector Switch was not placed in the test position, resulting in a Train A Safety Injection..The.cause for this event j

was cognitive personnel error by not placing the Mode Selector switch in test. High head injection occurred through valve 2SI8801A, pressurizer level increased from twenty-five.to forty percent and RCS pressure increased from l

three hundred forty five to three hundred fifty five pounds.

l Emergency procedure 2BEP-0 was entered upon receiving the Safety Injection signal and exited when plant conditions.were stabilized. Licensing Event Report (LER) 87-016-00; Docket No. 50-455 also covers this event.

l This is the first safety injection that has occurred-on Byron Unit Two.
