REQUALIFICATION EXAMINATION REPORT, FORT CALHOUN STATION (FCS) i On November 18, 1986, NRC administered examinations to employees currently licensed to operate your FCS facility. At the conclusion of the examinations, l

the examination keys were provided to facility staff personnel and preliminary findings were discussed with those members of your staff identified in the enclosed report. As a result of these examinations, the FCS requalification training program was evaluated as unsatisfactory for the purposes of certifying the proficiency of a licensed operator to receive license renewal.

Further, licensed operators who have not been evaluated by NRC within the past 2 years must be removed from licensed duties until a proficiency examination can be administered by NRC. Licensed operators affected by this requirement have been identified to OPFD and the affected FCS personnel. Until further notice, all examinations to determine proficiency for continued performance of licensed duties and for license renewal will be prepared and administered by NRC.

On December 22, 1986, NRC met with OPPD management to review the FCS requalification training program deficiencies and proposed corrective actions.

OPPD followed up with a letter, dated January 23, 1987, committing to a schedule of specific corrective actions.

NRC concurs with the OPPD proposed corrective actions and schedule of implementation.
