HIGHLIGHTS OF WEEKLY INFORMATION REPORT WEEK ENDING AUGUST 29, 1986 North Anna. Units 1 and 2 - Core Power Update On August 25, 1986, the staff issued Amendment Nos. 84 and 71 for North Anna.

Units 1 and 2, which allows an increase of the rated core power level from 2775 MWt to 2893 MWt. The amendments will increase the electrical output for each unit by 32 MWe, approximately 4.5%.

Unusual Event At Salem 2 On August 26, 1986, with the unit at 100 percent power, Salem Unit 2 experienced a turbine trip / reactor trip and safety injection (SI) apparently resulting from troubleshooting steam generator level instrumentation. Approximately 30 seconds later, a false station blackout signal was received causing the vital buses to strip their loads. Com)onent Cooling Water Pumps were stripped; control room operators tien tripped all four reactor coolant pumps. Two out of three vital buses were picked up by the station emergency diesel generators (EDG). One EDG was out of service for preventive maintenance. The SI was terminated 5 minutes after trip. Natural circualtion was established following the loss of the reactor coolant pumps and subsequently the third vital bus and nonvital loads were reenergized from offsite power. Two reactor coolant pumps were returned to service. The unit is in cold shutdown. The resident inspector is on site and following licensee actions. Two additional inspectors and a manager have been dispatched from Region I to the site.

FBI Criminal History Rule On August 18th, Mr. William Mcdonald, Deputy EDO, (Information Resources Management), and representatives from NMSS, SEC, GC, and RM met with the FBI to discuss interface processing requirements, reimbursement procedures, and FBI comments on the draft NRC rule.

It was determined that electronic processing was impractical.

A subsequent meeting of NRC staff was held on August 19th to discuss draft procedures for handling and tracking the status of incoming fingerprint cards.

In a related matter, GC will pursue initiating an amendment to the FY87 Appropriations Act to enable NRC to receive fees from licensees, retain a portion of our costs and reimburse the FBI for theirs.

Sequoyah Fuels Corporation. Gore. Oklahoma On July 18, 1986, Sequoyah Fuels Corporation (SFC) requested approval to perform limited cleanup activities at the Sequoyah Fuel Facility near Gore, Oklahoma.

This request was based on the need to reduce liquid inventories to allow access to equipment for essential maintenance and repair.

Reducing these inventories would also allow SFC to resume decontamination activities which had been stopped because of lack of storage space for the liquid generated during cleanup.

After discussions with Region IV, EPA and the State of Oklahoma, the NHSS staff, agreed on August 22, 1986, to allow limited operation of the yellowcake digestion tanks, solvent extraction system, evaporators, and uranyl nitrate boildown tanks.

All of the activities allowed would have had to be performed irrespective of the Commission's decision on restart.

Region IV is monitoring these monitoring these operations which have been uneventful to date.

Prior to agreeing to these limited activities, Congressman Synar's office was notified of the action and a press release was issued.

OFFICE OF NUCLEAR REACTOR REGULATION ITEMS OF INTEREST Week Ending August 29, 1986 Shoreham At 1:00 a.m. on August 26, 1986, the Shoreham turbine was synchronized with the Long Island Lighting Company Grid. The plant is currently operating within the 5% power limit, generating approximately 19 MWe. Plant hotel loads are approximately 14 MWe. The licensee intends to operate in this mode for a few days, and then complete the low power test program.

North Anna, Units 1 and 2 - Core Power Update On August 25, 1986, the staff issued Amendment Nos. 84 and 71 for North Anna, Units 1 and 2, which allows an increase of the rated core power level from 2775 MWt to 2893 MWt. The amendments will increase the electrical output for each unit by 32 MWe, approximately 4.5%.

Seabrook, Unit 1 On August 25, 1986, Public Service Company of New Hampshire, pursuant to 10 CFR 50.57(c), filed a motion for authorization of fuel loading and precritical testing of Seabrook, Unit 1.

The motion states that a boron concentration of 2000 ppm will be maintained in the reactor coolant system, and to preclude inadvertent boron dilution, nonborated water sources will be isolated by mechanically locking closed the appropriate valves.

The staff has 15 days to respond to the motion, unless either the intervenor or staff request a delay.

NRC TMI-2 CLEANUP PROJECT DIRECTORATE WEEKLY STATUS REPORT FOR AUGUST 25 - 29, 1986 1.

DEFUELING Bulk defueling operation continues using heavy duty hydraulic chisel and small spade bucket tools.

Four fuel canisters have been filled to date since operations began August 12, 1986.

A 60,000 gallon feed and bleed was performed on the RCS to improve water clarity.

PLANT STATUS The reactor remains in long term cold shutdown, vented to the atmosphere. Core cooling is by natural heat loss to cmbient building atmosphere. The average incore thermocouple reading is 80 F.

The airborne radioactivity on the defueling platform above the reactor vessel is about 4.5 E-7 uC1/cc Tritium and 5.6 E-12 uCi/cc particulates, predominately Cesium-137 and Strontium-90. About 15 feet of water over the core region and 6 feet over the carousel holding the defueling canisters provide shielding.

ENVIRONMENTAL MONITORING US Environmental Protection Agency (EPA) sample analysis results show that TMI site liquid effluents are in accordance with regulatory limits, NRC requirements, and The City of Lancaster Agreement.

The Lancaster water sample taken at the water works river intake and analyzed by EPA consisted of a seven day composite sample taken August 10 - 16, 1986. A gamma scan detected no reactor related activity.

TMI water samples taken by EPA at the plant discharge (includes Units 1 and 2) to the river consisted of seven daily composite samples taken from August 9 - 15, 1986. A gamma scan detected no reactor related activity.

The EPA analysis of the NRC outdoor air sample for the period August 21 - 29, 1986 will be reported in the next report.

D OFFICE OF NUCLEAR MATERIAL SAFETY AND SAFEGUARDS Items of Interest Week Ending August 29, 1986 Policy Statement on Radioactive Waste Below Regulatory Concern The Commission policy statement concerning radioactive waste below regulatory concern should be published the week of September 1, 1986 in the Federal Register.

Waste Management attended an ad hoc industry meeting on August 21, 1986 in Columbia, Maryland.

Representatives of utilities and utility organizations (Electric Power Research Instituto Nuclear Utility Waste Management Group, Edison Electric Institute, and Atomic Industrial Forum) met to discuss the industry's interest in generic rulemakings to permit the disposal of particular very low-level waste streams from nuclear power plants (e.g.,

compacted trash, secondary resins or contaminated soils).

Waste Management briefed the attendees on the policy statement and staff implementation plan.

The attendees seemed to be very interested in both petitions for rulemaking and individual proposals for alternative disposal methods under 10 CFR 20.302.

They were also pleased about the Commission's decision to issue an advance notice of proposed rulemaking on this topic.

Meeting on Advance Notification of Indian Tribes On August 8, 1986, representatives of the Office of State Programs and the Office of Nuclear Material Safety and Safeguards met to discuss the possible development of an NRC Policy Statement to address advance notification of Indian tribes for waste and spent fuel shipments.

NMSS is incorporating a survey of the emergency response capability of selected Indian tribes in its proposed update of NUREG-1620, " Survey of Current State Radiological Emergency Response Capabilities for Transportation Related Incidents." Agreement was reached to hold development of the Policy Statement in abeyance until the update of NUREG-1620 is completed.

Georgia Power Company Materials License No. SNM-1967 was issued on August 21, 1986.

This license authorizes the receipt, inspection, and storage of low-enriched uranium fuel assemblies for eventual use in the Vogtle-Unit 1 electric generating plant.

The plant is located in Burke County, Georgia.

The following Significant Enforcement Actions were taken during the past week:

a.

A Rescission of Order Suspending Licenses was issued August 22, 1986 to Radiation Technology, Inc. (Rockaway, NJ). The licensee was issued an Order Suspending License (Effective Immediately) June 23, 1986. This action was based on numerous personnel violations, includ-ing violation of requirements regarding interlock systems; senior licensee managemaat knew or should have known of these violations; and providing false information to the NRC regarding the interlock system. The licensee responded to the Order in many submittals.

After careful consideration to the licensee's responses, several inspections, and the licensee's agreement to certain conditions previously discussed, it has been determined that there is reasonable assurance that operations can be conducted without undue risk to the health and safety of the public.

A Notice of Violation and Proposed Imposition of Civil Penalty was issued August 27, 1986 to Louisiana Power and Light Company (Waterford 3) in the amount of $50,000. This action was based on the changing of operational modes on December 16, 1985 with an inoperable train of the containment spray system.

Stone & Webster, New York, NY - inspection will be conducted to review the implementation of the Stone & Webster QA program, specifically those activities relating to piping / pipe support evaluation as part of the Cemanche Peak response (CPRT) design adequacy program.

Quality Assurance The Steering Committee from the Edison Electric Institute's Nuclear Supplier Quality Assurance Committee met with members of the Division of Quality Assurance, Vendor, and Technical Training Center Programs to discuss their program for cooperative audits of nuclear vendors. At present, 25 utilities are inco1ved in developing the program which includes developing standardized quality specifications as well as performing cooperative audits.

Performance Indicator Program The interoffice group on Performance Indicators met August 26, 27, 1986. Based on the completed trial program, the group selected the indicators to be recommended for NRC use. Definitions were aligned with INP0 definitions where overlap occurred.

Generic Issues Members of the Engineering and Generic Communications Branch parti-cipated in a meeting with the State of Illinois Department of Nuclear Safety that was held in Bethesda on August 28, 1986. The subject of the meeting was the potential state involvement in the review of components at nuclear power plants that are subject to the provisions of the ASME Code.

OFFICE OF NUCLEAR REGULATORY RESEARCH Items of Interest Week Ending August 29, 1986 Wide-Plate Test of Charpy Low-Upper Shelf Energy Steel On Friday, August 22, 1986, the seventh in a series of 18 planned wide-plate crack arrest experiments was carried out at NBS Gaithersburg. This program, part of the HSST program, is intended to develop fracture and arrest data in situations applicable to reactor vessel overcooling accident conditions and, in particular, pressurized thermal shock. The first six tests in the series used present-practice A533-B plate which has a Charpy upper shelf energy of approximately 120-130 ft-lbs. The present experiment, as well as the five other tests to follow in the second series, used a special chromium-molybdebum plate with a Charpy upper shelf energy of approximately 60 ft-lbs, simulating low upper shelf energy welds and plates found in some of our older reactor vessels.

The primary observation from this test, based on immediate post-test observa-tion of the fracture surface, was that a dynamic crack arrest occurred rather than a stable arrest, and the plate reinitiated immediately and did tear almost completely in half. This tearing behavior in the low energy upper shelf plate is quite different from the behavior in previous tests on present practice material having a high energy upper shelf; in former tests, stable arrest did occur after the run-arrest event and the plate did not tear in a ductile manner. Considering the relatively low net stress in the test plate of the low energy upper shelf material, following the dynamic run-arrest event, the final tearing of the plate could only be due to its low Charpy energy and hence its low resistance to ductile tearing.

A more positive observation was that the low energy upper shelf material initiated the original crack-run once the material was stressed above its Kic (level of fracture toughness for crack initiation). This observation means that even for such low energy upper shelf materials, the presently used PTS methodology is accurate for use of Linear Elastic Fracture Mechanics for crack initiation and up to the point of arrest, followed by quasi-static elastic-plastic fracture mechanics to judge the remaining adequacy of the vessel following dynamic arrest. However, this experiment does raise a concern about the ability of low-upper shelf material vessels to sustain their structural integrity during a PTS event under conditions where a present practice material vessel might crack but not fail.

It must be cautioned that this is very preliminary data and is only a small part of an integrated program addressing the entire issue of Charpy low-upper shelf energy material vessels.

In three weeks the eighth wide-plate test using this same material will have been conaucted.

In October, PTSE-2, the second pressurized thermal shock experiment, will also have been conducted, using the same material.

2 NRC Test FLHT-4 R. Van Houten notified us today by phone that this severe fuel damage test in the Canadian NRC test reactor was run successfully this morning at about 1:00 a.m.

The main purpose of the test was to measure hydrogen production at high temperatures during fuel degradation (liquefication and slumping). High temperatures were achieved and maintained for about 30 minutes and several hydrogen detection instruments measured the release of hydrogen throughout this period. PNL and AECL staff who 4

performed the test are reported to be very happy with the extremely smooth performance of the test. A more complete weekly report discussing this test will be prepared when Van Houten returns to the office next week.
