07/31/84 2 of 4 Each department head shall prepare a listing of procedures and instruc t io ns for which his organization is responsible for implementing. Be specific; for example file custodians, DCP.

series; pipe craftsmen, CPM 6.9, etc.

And consider the inter-facing procedures; for example electricians, CPM 13.1 ( fo r checkout of MSTE and completion of tool issue cards).

Only che listed items and their subsequent revisions need be reviewed by the department heads.

Care should be taken to ensure that all of the interfacing procedures are included.

These lists shall be submitted to the Field Administrative Group.

NOTE:

This procedure does not apply to the QA Department.

QA training is administered in accordance with the implementing QA procedures.

1.2 SCOPE 1.2.1 This procedure ap' plies to all department heads who are required to per form sa fety-related work and/or administrative activities (e.g., pr oc ureme nt, calibration, document control, etc.).

1.3 GENERAL DISCUSSION 1.3.1 All craf t and non-cra ft personnel shall be made aware of procedures / instructions and subsequent revisions for which they are responsible for implementing.

2.0 PR OCEDURE 2.1 REVIEW OF PROCEDURES / INSTRUCTIONS AND REVISIONS 2.1.1 Department heads and/or their designees shall review all newly published procedures and instructions, and revisions and DCN's to those procedures / instructions identified in accordance with paragraph 1.1.

The purpose of this review is to determine if members of their department require knowledge of the procedure, revision, or change notice to perform their tasks properly.

2.2 TRAINING NOT REQUIRED If, on review of the procedure or revision, it is determined training is not required, the department head will document the review and determinination by use of the fonn shown in At tach-ment 1.

07/31/84 3 nr 4 2.3 TRAINING REQUIRED 2.3.1 If training is required, those to whom the procedures or revisions apply must be identified and the training conducted immediately by supervisors designated by the department head.

2.3.2 Training Method The information relateh to the procedure or revision may be presented by either of the following methods:

1.

Orally by the supervisor.

2.

Copies provided each person to read.

Irrespective of the method selected, the change will be discussed to assure common understanding or interpretation. This discussion

'will be conducted by the supervisors and/or department heads.

All training will be documented by use of a Training Record (Attachment 2).

3.0 DOCUMENTATION 3.1 Particular care must be exercised to properly and accurately identify the procedure and revision and/or DCN under discussion.

The procedure nomenclature provided in Attachment 3 should be used when completing the Training Records.

3.1.1 The Training Review Record (Attachment 1) will be initiated and -

signed by the department head when it is determined that training is not required.

3.1.2 The training record will be prepared to document any training determined to be necessary as a result of the procedure or the revisio n.

3.1.3 The training record will be prepared with the below listed infor-mation by the department head conducting the trainir.g.

1.

Date of training.

2.

Procedure number, revision number, and DCN number.

3.

Signature, badge number, and social security number o f a ttending personnel.

3.1.4 The instructor will sign the record, indicate the hours of training, time training session began and the department involved.

An extension sheet may be used for large groups. A copy of the computer printout submitted to the various departments may be signed by those receiving the training and attached to training record and used instead of the Training Record.

The Field Administrative Group shall review the Training Records for computer input capability. The records shall then be transmitted to the computer personnel for input and review. The Field Administrative Group will be provided with a printout which will serve as the record for the training.

4.0 RESPONSIBILITIES 4.1 Department heads and supervisors are assigned prime responsibility for compliance with this procedure.

4.2 The Field Administrative Group shall be responsible for assistieg in the training ef fort and monitoring compliance.

4.3 The Field Administrative Group shall be responsible Sor notifying departments that fail to submit copies of the appropriate docu-mentation within ten (10) working days following issue of the cha nge.

5.0 REINDOCTRINATION DOCUMENTATION When reindoctrination in a procedure is required in cases such as dispositions to NCR's or CAR's, the reindoctritation should be documented in accordance with this procedure. The Training Record should re ference the NCR, CAR or other reasons for the reindoc trina t io n.

Contrary to the above, on June 19, 1984, the NRC inspector observed that Alarm 2.6 (which monitors Unit i electrical penetration nitrogen pressure) was in an alarm condition and determined that the actions required by l

Alarm Procedure X-ALB-138 had not been performed.

Contrary to the above, on July 14, 1984, the NRC inspector visually inspected and determined that fire rated penetration seals had initially been properly installed by BISCO, but had subsequently been removed by unknown construction personnel who did not process a penetration seal removal request (PSRP.) as required by Brown & Root, Inc. (B&R) Procedure CP-CPM-6.10, Revision 11, dated, February 16, 1984. Specifically, Internal Work Release 0217RA and "As-Built" Drawing SG-873-108T-1 show authorized rework and actual configuration on the referenced drawing; however, Seal Trace 5003 had subsequently been broken and damaged when additional I cable was run throuGh this seal. .o I, [ The NRC inspedtor determined from review of documents and discussions with j . cognizant.licel.see personifel that between February and May of 1983 three of the four Unit J Westinghouse instrument power supply inverters failed. The failure of each of'the inverters was caused by a grounded secondary winding of the inverter ferroresonant output transformer (IT on Drawing 4950C67, Sheet 4). In February 1983 Inverters IVIPCl and IV1PC3 failed and in May 1983 Inverter IVIPC2 failed. Licensee personnel initiated test deficiency repoi*ts and NCRs to document these failures and obtain new transformers to correct the defect. The licensee also initiated action through Westinghouse (the inver*er supplier) to determine the cause of the failures, and on November 30, 1983, General Electric (the transformer vendor) issued a fault analysis report which concluded that the cause of the transformer failure was'due to the lack of sufficient securing of the center leg to prevent the leg fromishifting and vibrating due to the magnetic forces encountered. The vibration abrades the coil insulation and, in time, the coil insulation is penetrated causing the coil to short to the core. Thelicensee'shippeh'theaffectedtransformerstoGeneralElectricfor modifications which are designed to correct the defect. A total of 12 transformers have been shipped by the licensee to General Electric for these modifications. The licensee receive ^d the General Electric fault analysis report on January 16, 1984, and made a verbal 10 CFR Part 50.55(e) report (CP 84-04) to the NRC. The written 10 CFR Part 55.55(e) report was subsequently issued on February 13, 1984. The fact that three out of four Unit 1 safety-related inverters suffered a common cause failure that was identified by the licensee but not reported within the time frame required by10CFRPart50.55(e)isanapparentviolation(445/8422-02). 5. Review of OA Manuals and Imple'menting Procedures The NRC inspector identified contractors [B&R, Bahnson, Chicago Bridge and Iron (CB&I)/ Grinell, and Westinghouse] which have performed construction - work on site. B&R, as construction manager / constructor, has provided TUGC0 with construction services and was responsible for the QA program for A9ME Code work. Westinghouse, as nuclear steam supply system supplier (NSSS), has provided TUGC0 with the nuclear steam supply system by con-ducting engineering, design, procurement, and fabrication services for the NSSS and initial supply of nuclear fuel in accordance with a Westinghouse Topical QA Report. Bahnson is responsible for heating, ventilation, and air conditioning. CB&I constructed the containment liner. Grinell 11 doing the fire protection work. TUGC0 and each of the contractors -established QA programs consisting of manuals and implementing procedures for controlling site construction activities. ~ e 4 4 p n 4
