Subject:

NOTICE OF VIOLATION (Inspection Report No. 50-155/85014).

This refers to the routine safety inspection conducted during the period

July 30, 1985 through October 31, 1985, at Big Rock Point Nuclear Plant of

ac.tivities authorized by NRC Operating License No. DPR-6.

The inspection

identified several violations of NRC requirements.

An Enforcement Conference

was held with you and nembers of your staff on December 5, 1985, to discuss

the violations, their causes, your corrective actions, and NRC enforcement

options.

The violations, identified as a result of two separate incidents, were

of particular concern because they involved numerous examples of personnel

errors, failures to follow procedures, and a significant lack of management

control over plant maintenance and repair activities.

On September 10, 1985, a maintenance supervisor received a work authorization

to repack the steam isolation valve for the steam seal regulator and air ejector,

CV-4104.

However, on September 11, 1985, as a result of a failura to positirely

identify the valve, repairmen disassembled and repacked CV-4106, the wrong valve.

By fortunate coincidence, the isolation for CV-4104 also provided isolation for

CV-4106 thereby averting possible serious consequences.

On September 25, 1985, the improper identification of a valve by supervisory

personnel, as well as repairmen, resulted in individuals disassembling the wrong

valve on a system that had not been depressurized.

As a result, pressure from

the refueling shield blew out the valve's internals and discharged approximately

1,400 gallons of water onto the pump room floor.

Because the repairmen failed

to promptly notify the control room, it took an estimated 35 minutes before the

leak was isolated.

After reviewing the inspection findings, the NRC staff has concluded that these

two occurrences resulted directly from a failure of supervisory personnel and

repairmen to follow administrative and maintenance procedures.

that the consequences of these events were not more serious.

It is essential

that in the future, licensee management take an active role in ensuring that

administrative and maintenance procedures are understood and followed.

In accordance with the " General Statement of Policy and Procedure for NRC

Enforcement Actions," 10 CFR Part 2, Appendix C (1985) the violations have been

classified in the aggregate as a Severity Level III problem.

Normally a civil

penalty is considered for a Severity Level III violation or problem.

However,

after consultation with the Director, Office of Inspection and Enforcement, I

have decided that a civil penalty will not be proposed in this case.

In making

this decision, I have considered your prior good performance in the general area

of concern and your extensive and comprehensive corrective actions that you

described during the December 5,1985 Enforcement Conference.

These corrective

actions included:

counseling of personnel, component identification, training,

upgrading of procedures, disciplinary action, and involvement by corporate

management in control of maintenance activities.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your response,

you should document the specific actions taken and any additional actions you

plan to prevent recurrence. After reviewing your response to this Notice,

including your proposed corrective actions, the NRC will determine whether

further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.

In accordance with Section 2.790 of the NRC's " Rules of Practice," Part 2

Title 10, Code of Federal Regulations, a copy of this letter and the enclosures-

will be placed in the NRC's Public Document Room.

The responses directed by this Notice, are not subject to the clearance procedures

of the Office of Management and Budget as required by the Paperwork Reduction

Act of 1980, PL 96-511.
