AND NOTICE OF VIOLATION

Dear Mr. Tulon:

,

On September 22,1997, the NRC completed a .nspection at your Braidwood Unit 1 and Unit 2

reactor facilities. The enclosed report presents the results of that inspection.

During the 6-week period covered t , this inspection, your conduct of activities at the Braidwood

facilities was generally characterized by safe'y-conscious operations, sound engineering and

maintenance practices, and careful radiological work controls. Operators promptly recognized

and correctly responded to a failure of the 28 feedwater pump speed controller card, minimizing

the esulting plant transient. Additionally, the maintenance activities on the 1B main steam

isolation valve were well planned and executed and the different functional groups demonstrated

excellent team work in the performance of the repairs.

However, we are concemed about the failure to follow procedures and component

mispositionings. The 2B containment spray eductor drain valve (2CS0128) was found closed as

required, but unlocked on August 20. The valve had been verified locked by two operators on

August 12 and by two different operators on August 14. Your root cause evaluation found that

operators were not properly using the administrative controls for maintaining status of locked

valves and that operators were not following administrative procedures to verify the position of

locked valves. Mispositioned components are a continuing concem that has been discussed in

previous inspection reports improper verification of valve position is also a recurring problem.

Your corrective actions for this problem have not been fully effective as evidenced by the fact

that Braidwood Station has averaged about four mispositioned components per month for each

of the last 5 months. The violation is cited in the enclosed Notice of Violation (Notice) and the

circumstances surrounding the violation are described in detail in the enclosed report.

The NRC inspectors identified that the post modification tests on tha engineered safety feature

switchgear and miscellaneous electric equipment room ventilation systems, in July 1996, were

performed without an approved test pncedure. The testing on the engineered safety feature

switchgear ventilation system failed to identify that the modification had been installed

improperly. We are concemed that your root cause investigation failed to identify that the test

to ciarify test documentation requirements and training of engineering personnel on the need for

procedure adherence and test documentation. The violation is citou as item 2 in the enclosed

Notice, and the circumstances surrounding the violation are describect in detail in the enclosed

report. We note that this violation had been corrected and those actions were reviewed dunng

this inspection. Therefore, no response to this matter is requested.

Regardmg item 1 in the enclosed Notice relating to improper value positioning verification, please

note that you are required to respond to this letter and should follow the instructbns spedfled in

the enclosed Notice when preparing your response. Your response should not only detail the

actions to be taken for the specdic issues identdied, but should also be broad!y focused on what

actions yw plan or have taken to ensure that your conective actions process is effective. The

NRC will use your response, in part, to da.ormine whether further er.!orcemord action is

- necessary to ensure compliance with regulatory requirements.
