Region 111 801 Warrenville Road Lisle, Illinois 60532-4351 Portsmouth Gaseous Diffusion Plant (PORTS) - Docket No.70-7002 - 10CFR95.25 - Event Report 98-20 In accordance with 10 CFR 95.25, this report summarizes the actions taken as a result of the discovery of an unattended security container found open. Enclosure 1 lists the actions taken.

On December 17,1998, at approximately 1215 hours0.0141 days <br />0.338 hours <br />0.00201 weeks <br />4.623075e-4 months <br />, the door to the reproduction area vault V-282, an authorized classified matter security container, was discovered to be open and unattended. The Security Investigation into the incident determined that a reproduction representative left the vault without securing the vault door or ensuring that another authorized custodian was present. The investigation also determined that an additional incident involving the same individual, occurred on December 17,1998, between 1500 and 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br />. Since the classified material in the X-100 Reproduction Vault V-282 was locked in a separate authorized security container located within the vault during both incidents, no unauthorized access was gained to the container and no classified information or material was compromised. An inventory of the locked security container was conducted on December 18,1998. The inventory verified that the classified material was present and undisturbed.

The reproduction personnel who discovered the unattended vault reported this incident to their group manager. However, this condition was not reported to the Organization Manager or the Plant Shift Superintendent until December 18,1998. As a result, the discovery of this condition was not reported to the NRC within one hour of discovery as required by 10 CFR 95.57(b). This condition was reported to the NRC on December 18,1998, at approximately 1300 hours0.015 days <br />0.361 hours <br />0.00215 weeks <br />4.9465e-4 months <br />.

On December 18,1998, the combination to the X-100 Reproduction Vault was changed.

Crew briefings on vault security control requirements were initiated for Administrative Support personnel to re-emphasize proper vault control.

On December 22,1998, the reproduction representative was issued a security infraction. The reproduction representative was reminded of his responsibility for the control and protection of classified information.

On January 4,1999, a crew briefing was conducted with reproduction personnel emphasizing the procedural requirements to promptly report security problems to the PSS.
