Dear Mr. Merschoff:
Entergy Operations, Inc. is hereby notifying the NRC of two items associated with Operator medical conditions. The attachments contain personal information which is not to be disclosed to the public. This cover letter does not contain personal information when not accompanied by the attachments.
First, in accordance with 10CFR55.25, Entergy Operations, Inc, requests the removal of the medical restriction currently in effect associated with the Reactor Operator license of Jeffery C. Baxter, License No. OP-42417 and Docket No. 55-40657. Thir request is based on medical condition Wiprovement. See the attached NRC form 396 and other applicable form.
Secondly, we are notifying the NRC of a temporary medical condition affecting Waterford 3 Reactor Operator Dyne Kevin Cassidy, License No. OP-42625 and Docket No. 55-40959. See the attached NRC Form 396 and letter-to-file describing 4 the condition. This item is bein. provided for information purposes only. No NRC JI O.,
action is requested at this time regardir:g this second item. The NRC will be promptly notified when thi condition and/or the condition classification changes.
