Gentlemen:
This refers to the routine safety inspection conducted by Messrs. M. Jordan,
J. Bjorgen, R. Kopriva, and R. DeFayette of this office from April 15 through
May 9,-1986, of activities at LaSalle County Station, Units 1 and 2, authorized
by Operating Licenses No. NPF-11 and No. NPF-18 and to the discussion of our
findings with Mr. G. J. Diederich at the conclusion of the inspection.
The enclosed copy of our inspection report identified areas examined during
the inspection. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, observations, and
interviews with personnel.
During this inspection, certain of your activities appeared to be in violation
of NRC requirements, as specified in the enclosed Appendix. A written response
is required. These violations are indications of a continuing problem of
personnel errors, a problem which we brought to your attention in our letter
of November 22, 1985. .While we recognize that the number of Licensee Event
Reports attributable to' personnel error has declined since that time, the rate
of personnel errors is still excessive.
It is of particular concern that a
number of the errors discussed in the enclosed report occurred during fuel
handling operations, given the extensive experience possessed by Commonwealth
Edison in this area and the success with which these operations have been
conducted at LaSalle previously. Because of our continuing concern in this
~
area and recognizing that Unit 2 will begin a refueling outage this Fall, we
request that your response to the enclosed Notice of Violation address not
only the specific problems identified, but also speak to the general issue
of personnel error rate reduction. We also request that you address this issue
specifically in the next scheduled monthly meeting on the status of
10 CFR 50.54(f) issues.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures will be placed in the NRC's Public Document Room.
The responses directed by this letter (and the accompanying Notice) are not
subject to the clearance procedures of the Office of Management and Budget as
required by the Paperwork Reduction Act of 1980, PL 96-511.
