1. Routine inspection ;

a.
Insoeglion Scoos
- On November 25,= 1907 the inspectors conducted an entrance meeting, toured the
'
licensee's new facility located in Trainer, Pennsylvania, and reviewed -
documentation. On December 8,1997, OSL Inspection, Inc., conducted .
- radiography at a temporary job site located at a company near Nev' irk, Delaware.
.
.-The NRC inspectors, the Licensee's Radiation Safety Officer (RSO), and' assistant
!
RSO (ARSO) observed two radiographers arrive on site, establish restricted ' area
boundaries, conduct a radiological survey of the boundary while performing an
exposure. The inspector also conducted an exit meeting with the RSO.

LOn December 8,1997 when the two radiograpNrs arrived at the temporary job
'
site the radiographe_rs exited the vehicle and entered a building. The radiographers -
.
-left the vehicle about 50 to 75 feet away from the vehicles in which the inspector
'
'
and.RSO were sitting, in full view. After a few minutes the inspector approached
.
the radiographer's vehicle and found the driver's door unlocked. The inspector
then checked the door to the darkroom at the back of the vehicle. The door at the
>
back of the vehicle was locked.1The inspector returned to the drivea's door and
opened it again and notice that the keys were in the ignition. At that point the
"
inspector called the RSO over to the vehicle and discussed the situation.' The
ARSO took the keys from the ignition.
The inspector asked if the any of the keys on the key ring would open the back of
the vehicle and the overpack which secures the radiography exposure device. The
ARSO indicated that the keys would open the back of the vehicle and overpack.
- The ARSO proceeded to open the back of the vehicle and the overpack. The
inspectors asked the ASRO and RSO what was expected of the radiographers
when they left the vehicle. They both indicated that the vehicle should have been
'
locked and the keys should be with the radiographers. They also indicated that the
radiographers when asked will probably respond by saying that when in a refinery
i they are required to leave the keys in the vehicle. When the radiographers
- returne.d the inspector, and RSO both separately asked the radiographers why they
left the keys in the vehicle. Both radiograhpers independently stated that it was a

force of habit because when they perform radiography at a refinery they are-
required to leave the keys in the vehicle. This temporary job site was not located
,
L at a. refinery.

The RSO continued to question the radiographers and reinforce what is required of
them while transporting licensed material. The RSO further stated that he will
reinforce the requirement to secure the vehicle during upcoming scheduled
' training,

The RSO was very knowledgeable of the licensee's proceduros for safe use of

licensed rat'ioactive material,

The finding that, on December 8,1997, the licensee did not secure from

unauthorized removal or limit access to a radiography exposure device containing
!
cpproximately 74 curies of Iridium 192 in a sealed source located in a vehicle
"
which is an unrestricted area, nor did the licens>)tt Jontrol and maintain constant
i
survell.ance of this licensed material as required'in 10 CFR 20.1801 is an apparent
,
violation of 10 CFR 20.1801,
