United States Nuclear Regulatory Commission SERIAL: GDP 97-1031 Region Ill 801 Warrenville Road Lisle, Illinois 60532-4351 Paducah Gaseous Diffusion Plant (PGDP)- Docket No. 70-7001 - 10CFR95.25 Report In accordance with 10CFR95.25(h), this report summarizes the actions taken as a result of the discovery of a vault door unlocked and unattended in Building C-100. Attachment 1 is the
" actions taken" to preclude recurrence.
On November 29,1997, at 1915, a Security Police Officer (SPO) making routine security checks of classified vaults discovered that the Building C-100 first floor " print vault" doors were closed, but unlocked. The SPO attempted to spin the combination dial; however, it would not spin. He then operated the vault door handle and was able to open the vault door. The SPO notified the Police Shill Commander (PSC) who responded to the scene and noted that the SPO was standing by the vault with the door in the open position. The responding Commander was not the same individual responsible for locking the vault. The vault door was subsequently closed and locked.
The area was checked and there were no other individuals present. The Plant Shifl Superintendent (PSS) was notified, as was the vault's custodian. At approximately 2100 the

vault custodian inspected the vault area and determined that nothing had been tampered with to 'g g
the best of his knowledge. The highest classification of the information stored in the vault is Confidential, Restricted Data (CRD); therefore, a document inventory was not conducted.
On December 2,1997, the PSL responsible for locking the vault on November 29,1997, was interviewed relative to the incident. IIe stated that he opened the C-100 vault at 1556 for purposes of allowing a firefighter from Fire Services to conduct a firewatch. At 1558 the l
Commander closed and locked the vault doors, lie stated that aller closing the vault doors he pulled the handle on the door into the upright position, released the " butterfly" located on the face of the combination dial, rotated the combination in both directions and t.ttempted to pull the handle down to verify that the door was locked. After completion of the locking and double checking activities, both employees departed the area. At the time of the firewatch
- and subsequent securing of the vault, there were no other employees in the immediate vicinity.
On December 2,1997, the firefighter was interviewed relative to the incident. Ile stated that he arrived at the C 100 vault accompanied by the PSC for purposes of completing a firewatch of the vault's interior, lie stated that after completing the check he saw the PSC close the vault doors, spin the combination dial and grab the vault door handles. The firefighter said that as a double-l check he grabbed the vault door handles and shook them.110 wever, he did no t spin the combination dial lie stated that he was being watched while performing thedouble check by the PSC.
On December 2,1997, it was determined that a routine check of the C 100 vault was conducted by an SPO on November 29,1997, at 1628. On December 3,1997, the SPD responsible for conducting this check was interviewed. The SPO stated that during his che:k of the vault, he pulled on the vault door handles and rotated the combination dial several times. The SPO stated that the vault was secure at the time he made the check at 1628. The time of the check of the vault door was confirmed by a review of an extract of the print out reflectiag the time the data acquisition unit (DAU) was passed over the vauh's data strip.
On December 3,1997, each custodian of the C-100 vault was asked whet'aer they were on the plant-site on November 29,1997. Each stated that they were not on-site on the day in question, with the exception of the custodian that was called into the plant at 2100 after the vault was discovered unlocked.
The PSS log, operator's log located in the cascade coordinator's office,imd the C-100 first floor log access list located in the vaun area, were reviewed to detennine whe ther on November 29, 1997, authorized access to the vauh/ vault area had been noted. ' ne of the logs indicated ateess during the time in question (1556-1915), with the exception of the C 100 first floor log access list which contained the initials, time-in (1556) and time-out (1558), of the firefighter.
A review of the SF702," Security Container Check Sheet" confirms the vault was opened (1556),
closed (1558) and checked (If 58). ' One additional entry conSrms the c astodian inspection (opened 2114, closed 2120 and checked 2120).
Interviews with the individuals involved in locking and double-checkir g the vault indicate that proper steps were followed. The only deviation was during double-checking activities when the combination dial was no: rotated. Ilowever, the vault was confirmed locked during the routine police check of the vault at 1628. An individual could have possibly opened the vault

subsequent to the check at 1628 and prior to the discovery at 1915, but there is no evidence to i
substantiate this supposition.
