APPENDIX A NOTICE OF VIOLATION Wolf Creek Nuclear Operating Corporation Docket:
50-482 Wolf Creek Generating Station Operating License:
NPF-42 During an NRC inspection conducted on January 4 through February 14, 1988, a violation of NRC requirements was identified.
The violation involved a failure to comply with licensee's plant change procedure.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:
Failyre to Comply with Plant Change Procedure Criterion V to Appendix 8 of 10 CFR Part 50 requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings and shall be accomplished in accordance with these instructions, procedures, or drawings.
Administrative Procedure (ADM)01-022, Revision 13. "Authorization of Changes, Tests, and Experiments," requires that plant changes be reviewed for a reduction in the margin of safety and that this review be documented by completing the "Wolf Creek Nuclear Safety Evaluation Checklist."
Contrary to the above, on February 1,1988, it was discovered that a plant change had been made which affected residual heat removal (RHR) system piping and no review had been conducted to determine if a reduction in the margin of safety had been created.
The change identified involved steel test flanges hanging by electrical cable tie wraps from RHR system Valves EJ V-172, 174, 175, and 178.
The RHR piping involved is Seismic Category I and no evaluation of the additional seismic loading was performed.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR 2.201, Wolf Creek Nuclear Operating Corporation is hereby required to submit to this Office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:
(1) the reason for the violations if admitted, (2) the corrective steps which have been taken and the results achieved, (3) the corrective steps which will be taken to avoid further violations, and (4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
