i' Since the pump was declared inoperable, the unit was placed in the action state-ment of Technical Specification 3.1.2.4, which requires two makeup pumps in Modes 1,'2, 3 and 4.

The cause of this occurrence is per-Designation of Apparent Cause of Occurrence:

Work was done on the pump on August 8,1978 under Maintenance Work sonnel error. to investigate inboard bearing temperatures that would not stabilize.

Order 78-1860 After disassembly'and-inspection of the bearing, no abnormal conditions were found and the bearing was reassembled.

On August 13, 1978, an equipment operator found a low oil level in the reservoir sight glass for the pump oil system. Upon investigation by maintenance personnel, Further a fine mist of oil was being discharged around the inboard deflector disc.

78-2051 revealed small quantities of a investigation under Maintenance Work Order sealant -(RTV-106) around the inboard deflector disc which prevented a proper oil seal.

Under Maintenance Work Order 78-2051, the pump was cleaned, J
