TOLEDO EDISON COMPANY DAVIS-BESSE UNIT ONE NUCLEAR POWER STATION SUPPLEMENTAL INFORMATION FOR LER NP-33-78-107 DATE OF EVENT: August 13, 1978 FACILITY: Davis-Besse Unit 1 IDENTIFICATION OF OCCURRENCE: Makeup Pump 1-2 declared inoperable due to excessive inboard deflector disc oil leakage 4
The unit was in Mode 1, with Power (MWT) = 2080, Conditions Prior to Occurren'e:
and Load (MWE) = 292.
Ai 1018 hours0.0118 days
0.283 hours
0.00168 weeks
3.87349e-4 months
on August 13, 1978, Makeup Pump 1-2 was Description of Occurrencq:
dec2 red inoperable due to excessive oil leakage from the inboard deflector disc.
i' Since the pump was declared inoperable, the unit was placed in the action state-ment of Technical Specification 3.1.2.4, which requires two makeup pumps in Modes 1,'2, 3 and 4.
t I
The cause of this occurrence is per-Designation of Apparent Cause of Occurrence:
Work was done on the pump on August 8,1978 under Maintenance Work sonnel error. to investigate inboard bearing temperatures that would not stabilize.
Order 78-1860 After disassembly'and-inspection of the bearing, no abnormal conditions were found and the bearing was reassembled.
On August 13, 1978, an equipment operator found a low oil level in the reservoir sight glass for the pump oil system. Upon investigation by maintenance personnel, Further a fine mist of oil was being discharged around the inboard deflector disc.
78-2051 revealed small quantities of a investigation under Maintenance Work Order sealant -(RTV-106) around the inboard deflector disc which prevented a proper oil seal.
l There was no danger to the health and safety of the public Analysis of Occurrence:
or to unit personnel. The redundant makeup pump was operable.
Corrective Action
Under Maintenance Work Order 78-2051, the pump was cleaned, J
ieassembled, and restored to operability.on August 16, 1978 at 0245. hours. This A note has is within the 72 hour8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
time limit of Technical Specification 3.1.2.4.
- - been added to procedure MP 1410.22, " Removing and Replacing Makeup Pump Bearings, Mechanical Seals and End Covers" to apply sealant sparingly to the horizontal bear-
'ing housing joints.
There has been one previously reported makeup pump oil leak but the
' Failure Data:
cause of the occurrence was a design deficiency.
See Licensee Event' Report NP-33-77-73 for further details.
