APPENDIX A NOTICE OF VIOLATION Arkansas Power & Light Company Dockets:

50-313/85-27 50-368/85-28 Arkansas Nuclear One, Units 1 & 2 Licenses: DPR-51 NPF-6 During an NRC inspection conducted during the period December 1-31, 1985, violations of the NRC requirements were identified. The violations involved failure to maintain a Unit 2 manual valve locked as required and failure to establish consistent procedures for the operation of the outside air dampers on the control room emergency recirculation and filtration units.

In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1985), the violations are listed below:

A.

Unit 1 Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained covering the activities recommended in Appendix ' A' of Regulatory Guide 1.33, November 1972.

Appendix ' A' of this Regulatory Guide includes procedures for auxiliary building heating and ventilation.

Unit 2 Technical Specification 6.8.1 requires that written procedures be established, implemented, and maintained covering the activities recommended in Appendix ' A' of Regulatory Guide 1.33, Revision 2, February 1978.

Appendix ' A' of this Regulatory Guide includes procedures for control room heating and ventilation.

Contrary to the above, on December 6,1985, the licensee had not established consistent procedures for operation of the outside air dampers on the control room emergency recirculation and filtration units during a chlorine event.

This is a Severity Level IV violation (Supplement I.E) (313/8527-01; 368/8528-01).

B.

Unit 2 Technical Specification 6.8.1 requires that, " Written procedures shall be established, implemented, and maintained covering... a.

The applicable procedures recommended in Appendix 'A' of Regulatory Guide 1.33...."

Plant Operating Procedure 2101.01, " Plant Pre-IIeatup and Pre-Critical Checklist," has been established in accordance with this Technical Specification.

. Attachment E " Category E Valve Position Verification," of Plant Operating Procedure 2101.01 requires that manual valve 251-11B be locked open for critical plant operations. 2SI-11B is the discharge crossconnect valve between the 'B' and 'C' high pressure safety injection pumps.

Contrary to the above, on December 17, 1985, with the plant operating at approximately 30% power, the NRC inspector found that manual valve 2SI-11B was open as required but was not locked.

Specifically, the locking chain for this valve was broken. This condition would allow chain removal without disturbing the lock, making the lock ineffective and allowing the valve to be operated.

This is a Severity Level V violation (Supplement I.E.) (368/8528-02).

Pursuant the provisions of 10 CFR Part 12.201, Arkansas Power and Light Company is hereby required to submit to this Office within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation:

(1) the reason for the violation if admitted, the corrective steps which have been taken and the results achieved, the corrective steps which u.11 be taken to avoid further violations, and the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time. No written response is required for the violation involving failure to maintain a Unit 2 manual valve locked, since adequate corrective action was taken and confirmed during this inspection.
