The report must be mailed to:

Nebraska Department of Environmental Control Permits and Enforcement Section P.O. Box 94877 State House Station 301 Centennial thll South Lincoln, Nebraska 68509 within 5 days after date of Non Compliance.

t 2.

Complete the heading, including permittee name,' NPDES Permit Number, and the date the sample was taken.

3.

List each parameter (BOD, TSS, pH, etc.) as listed in your NPDES Permit, showing the limits in the permit.

Flow data for day of sampling.is recuired.

4.

Enter the results of the analysis on the right side of the report.

5.

Complete sample type and the frequency of analysis used.

6.

Use blank section on report to give written comments as to the reason for Non Compliance.

7.

Have authorized agent sign report at bottom prior to railing..
