SUBJECT: NRC INSPECTION REPORT 50-298/97-02 AND NOTICE OF VIOLATION
Dear Mr. Horn:
An NRC inspection was conducted on January 12 through February 22,1997, at your
Cooper Nuclear Station facility. The enclosed report presents the scope and results of that
inspection.
Based on the results of this inspection, the NRC has determined that two violations of NRC
requirements occurred. These violations are cited in the enclosed Notice of Violation
(Notice) and the circumstances surrounding the violations are described in detail in the
enclosed report. Please note that you are required to respond to this letter and should
,
follow the instructions specified in the enclosed Notice when preparing your response. The
NRC will use your response, in part, to determine whether further enforcement action is
necessary to ensure compliance with regulatory requirements.
The first violation identifies three examples of maintenance technicians failing to properly
implement a procedure. The second violation describes a failure to implement appropriate
corrective actions.

The NRC is concerned with the failure to properly implement a maintenance procedure
because procedural adherence has been an ongoing issue at Cooper Nuclear Station and
was discussed in the recent SALP report. Our concern with the failure to implement
appropriate corrective actions is that it indicates that some plant personnel still do not
understand the Problem Identification Report (PIR) process, even though plant management
has given it much emphasis this past year.

its enclosure (s), and your response will be placed in the NRC Public Document
Room (PDR). To the extent possible, your response should not include any personal
privacy, proprietary, or safeguards information so that it can be placed in the PDR without
