NRC INTEGRATED INSPECTION REPORT 50-309/96-12
Dear Mr. Frizzle:
On October 26,1996, the NRC completed an inspection at your Maine Yankee reactor
facility. The results of that inspection were discussed with Messrs. G. Leitch, D. Whittier,
R. Blackmore, and others of your staff at an exit meting held on October 31,1996. The
enclosed report presents the results of tFat inspection.

During the 6-week period covered by this inspection period, your conduct of activities at the
Maine Yankee facilities was safety focused. However, we continue to note plant material
condition problems challenging operator performance.
Based on the results of this inspection, two apparent violations were identified. The
violations involved failure to maintain the primary vent stack sampling system operable
during maintenance on October 4,1996, and failure to properly control safeguards
information on October 17,1996.
These violations are cited in the enclosed Notice of Violation, and the circumstances
surrounding the violations are described in detail in the enclosed report. Please note that
you are required to respond to this !etter and should follow the instructions specified in the
enclosed Notice when preparing ycur response. The NRC will use your response, in part, to
determine whethe. further enforcement action is necessary to ensure compliance with
regulatory requirements.
