4 ENCLOSURE 1 NOTICE OF VIOLATION Florida Power Corporation Docket No. 50-302 Crystal River 3 License No. DPR-72 y

The following violation was identified during.an inspection conducted 'on March 25-28, 1985. ~ The Severity Level was assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix-C).
10 CFR 50, Appendix B, Criterion V, and FSAR Section.1.7.1.5 require that
. activities affecting quality shall be prescribed by documented instructions, procedures or drawings...- and shall be accomplished in accordance with these p.
instructions, procedures or drawings..The pipe support drawing and modifi-cation drawing provide installation dimensions for piece "H" of pipe support y -
=MUH 556.
The " Piping Hanger - Final Inspection" requirements provided on
' inspection plan. number. WR-60571 for pipe support. MUH 556 required - QC.
verification that "the overall configuration and sizing.of the completed E
hanger assembly or modification conform. to the applicable approved engineering; drawing."
M Contrary to the above, completed, installed, modified, inspected and QC accepted pipe support MUH 556 piece "H" installation dimensions did not 3
" conform" with the applicable approved _ engineering drawings.. In addition, e
O unauthorized. loads (scaffolding) were being partially supported by the pipe 1
support.
No procedure or -instructions were available authorizing or f
prohibiting installation of scaffolding / staging on permanent -plant
. equipment.
This is a Severity Level IV violation (Supplement II).
. Pursuant to 10 CFR 2.201, you are required to submit to this office within 30
' days of the date of this Notice,.a written statement or explanation in reply, 1
including:
(1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results. achieved; (4). corrective steps which will be taken' to avoid
- further violations; and (5) the date when full compliance will be achieved.
, Security or' safeguards information should be submitted as 'an enclosure - to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d)
