NOTICE Of VIOLATION

(NRC Inspection Report No. 50-344/92-32)

This refers to the inspection conducted by Messrs. R. C. Barr, J. F. Melfi,

and K. E. Johnston of this office on October 27 through December 7, 1992.

The inspection included a review of activities authorized by NRC License No.

NPF-1. At the conclusion of the inspection, the findings were discussed with

those members of your staff identified in the enclosed report.

Areas examined during this inspection are identified in the enclosed report.

Within these areas, the inspection consisted of selective examinations of

procedures and representative records, interviews with personnel and

observations by the inspectors.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation-(Notice).

These violations involved contract personnel exceeding

Technical Specifications limitations on overtime work, inadequate access

controls for the Trojan issue warehouse, and inadequate corrective actions and

reporting related to identified leakage of containment airlock door ecualizing

valves.

Three non-cited violations are also discussed in the enclosec inspec-

tion report, consistent with Section VII.B of the NRC Enforcement Policy.

We

encourage you to strengthen your oversight of contractor activities that can

affect the performance of safety related equipment, and to assure that your

contractors establish adequate procedures and administrative controls.

Our inspectors also observed excellent response by your plant operators to the

November 9,1992, steam generator (SG) tube leak.

The operators correctly and

rapidly responded to plant indications and followed emergency procedures.

Their expeditious shutdown and depressurization of the reactor was very

timely, allowing only a very minor release of radioactivity.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

Notice, including your proposed corrective actions and the results of future-

inspections, the NRC-will determine whether further NRC cnforcement-action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of

this letter and the enclosures will be placed in the NRC Public Document Room.

The response directed by this letter and the enclosed Notice are not subject

to the clearance procedures of the Office of Management and Budget as required

by the Paperwork Reduction Act of 1980 Pub. L. No.96-511.

Should you have any questions concerning this inspection, we will be pleased

to discuss them with you.
