Dear Mr. Keppler:
On May 11, 1984, Commonwealth Edison Company notified Mr.
W. L.
Forney of NRC. Region III of a deficiency reportable pursuant to 10 CFR 50.55e, regarding Concrete Expansion Anchors (CEAs) installed through finished floors.
Reference (a) provided information concerning this matter to fulfill the thirty day reporting requirement.
Reference (b) expanded the scope to include CEA installations in surface repaired concrete.
For tracking purposes, this deficiency has been assigned number 84-07.
The purpose of this letter is to revise corrective actions discussed in Reference (b) regarding new installations in finished floors.
The status of corrective actions has also been updated.
This-letter is cc.isidered a final report.
' CORRECTIVE ACTION TAKEN l
For previous installations in finished floor areas (NCR L617),
'Sargent and Lundy has completed a walk down of the finished z
floor areas in order to map.the size and location.
At this time 313 CEA support assemblies have been identified, 258 are acceptable, 38 assemblies are being evaluated, and 17 are unaccepte.ble due to current loading criteria.
Deficiencies will be reworked in accordance with Sargent~and Lundy criteria.
For new installations in finished floor areas (NCR L617),

'will provide: for monitoring of the spacing of adjacent expansion; anchor assemblies.
Sargent and Lundy Specification'BR/BY/CEA,,has been revised (Rev. 20) to include thisu requirement.
Consequently, previously issued
- drawings will not be' revised.
Upon receipt of the contractor notification, Sargent and Lundy will provide the correct embedment length.
For previous installations in surface repaired concrete areas (NCR_L634), Sargent and Lundy is organizing a walk down which should be complete by October 26, 1984.
After evaluation, any deficiencies will be reworked in accordance with Sargent and Lundy requirements.
For new installations in surface repaired concrete greas (NCR'L634), all. contractors installing CEA's were sent a set
.of drawings showing main patches (repairs to areas which l
- have exposed rebar) on July 26, 1984.
As new major patches are made, revised drawings-will be transmitted to all CEA e
contractors.- Physical demarcation of major patched areas in the building will be done as soon as taping material arrives on site.
